EXHIBIT Signature: Owner or Authorized Agent 1 /r 5 980607 To all concerned, I have applied for this special permit to raise, train and show dogs. The breeds that I raise are of the toy breed, Chinese Cresteds and Chihuahuas. I have a great respect for the health and welfare of all animals and do what it may take to insure that they are taken care of. Dr. Roger Klingenberg is my veterinarian. We give the dogs their shots,however they are monitored by Sheepdraw clinic. Dr Kingenberg does give the rabbi shots and examines the dogs at that time. We also keep the animals on Heartgard ( a Heartworm prevention) year around. We plan at this time to convert one of the bedrooms into a dog room and in the future build and addition for them. I follow a very strict disinfectant program for the areas that the animals will be using. To this date I have not had any major problems with the dog's health. All of the dogs are also identified with a microchip. They are registered with both AKC and the Avid Microchip Company. The animals will be kept within the yard area of the property with a 5 Ft fence. There is not going to be a problem of the animals getting into other property owner's livestock. We do let the dogs out in the mornings and then return them to their kennels (dog crates) for the day with food and water. In the evening we also let them out to exercise. The animals are watered and fed also in the evening. As a rule they are in their kennels by 9:00 P.M. each evening. When we are at home during the day we let them run a little more than during the week(depending on the weather). I sincerely hope that I will be able to obtain this license to assure that my dogs will always have a place to live. I have contacted the surrounding property owners and obtained a signed document stating that they do not foresee any problems. I have also indicated that if a problem does arise, they should contact me and we would work the problem out together. 980607 USE BY SPECIAL REVIEW QUESTIONAIRE 1. How will storm water drainage be handled on the site? 6 980607 ANSWERS FOR SPECIAL REVIEW QUESTIONAIRE 1. The property is zoned for Agriculture and the surrounding neighbors are raising horses.. 2. The intent of this zone district as I understand is to provide a safe,healthy environment for any livestock that may be kept. 3. There is not any future plans at this for the next 20 years future re-zoning. 4. Both of the immediate neighbors raise or board horses. The neighbor on the East side also grows alfalfa. 5. A small portion is in the possible flood zone. After talking with various neighbors, during the floods of the past the water has never reached any closer than 850 feet. I understand that this is in the 100 year clause. 6. The land will not be affected with the raising of the dogs. 7. All areas that the dogs will be kenneled and the areas that they run in will be disinfected at regular intervals. The inside area for the dogs is kept disinfected at least twice daily. 8. The site will be used for the raising,breeding,training of show dogs. 9. The nearest structure would be a chicken/peacock kennel that is approximately 150 feet from where the dogs would be if they are in the fenced area of our property. 10. There will not be any employees at this site. I I. Russ and myself will be the only people at this site.. 12. We raise Chihuahuas and Chinese Crested dogs and have 1 MinPin. The number varies from time to time. Our current City license allows 22. I would like to acquire for a license allowing 30 animals even if I do not anticipate this many animals at any given time. 13. There will not be any operating and processing equipment utilized on site other than normal. 14. In the future we plan on a one room addition for the dogs and a garage for our personal use. 15. We currently have two vehicles. One Suburban and a Dakota Dakota Pickup. 16. There is currently a septic tank with a leach line. 17. There will be no stockpile of wastes on this property. 18. The only waste and debris will be the normal of any household. BFI will be the trash disposal company. 19. There will not be any construction on this site other than a new fence around the house and a garage and one additional room in the future. 20. There are trees, lawns and fencing for property. No additional landscaping is needed for our use of the property. We will however be doing a lot of yard work such as planting of grass,flowers and maybe a few trees. Only to improve the appearance of the property. 21. I don't anticipate any reclamation procedures necessary. 22. This is a personal hobby and enjoyment for animals. We're proud to insure that an animal will have a safe and healthy environment. 23. There will be a Fire extinguisher in various areas of the house for easy access. The nearest fire department if needed is the Greeley Fire Department... 24. The water is provided by North Weld County Water District. 25.
