RE: APPROVE PURCHASE OF SERVICES AGREEMENT AMENDMENT AND
AUTHORIZE CHAIR TO SIGN - NORTH RANGE BEHAVIORAL HEALTH
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Purchase of Services Agreement
Amendment between the County of Weld, State of Colorado, by and through the Board of
County Commissioners of Weld County, on behalf of the Human Services Department, and
North Range Behavioral Health, commencing January 2, 1999, and ending December 31, 1999,
with further terms and conditions being as stated in said amendment, and
WHEREAS, after review, the Board deems it advisable to approve said amendment, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Purchase of Services Agreement Amendment between the
County of Weld, State of Colorado, by and through the Board of County Commissioners of
Weld County, on behalf of the Human Services Department, and North Range Behavioral
Health be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is authorized
to sign said amendment.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 19th day of July, A.D., 1999, nunc pro tunc January 2, 1999.

Amendment to Agreement with North Range
Behavioral Health
Enclosed for Board approval is an amendment to the FY 1999 agreement
between North Range Behavioral Health (NRBH) and the Weld County Area Agency on
Aging. This amendment increases the Area Agency's award to NRBH by $9,000
bringing the total award to $32,737.00 for 1,800 contact units of peer counseling for 90
persons.
The purpose of this amendment is to provide funding for two (2) additional part
time peer counselors primarily to work with elders in south Weld County. Funds are
earmarked specifically for salary and mileage for two part time peer counselors.

PURCHASE OF SERVICES AGREEMENT AMENDMENT
THIS AGREEMENT, made and entered into this 14 day of July, 1999, by and between the Weld
County Division of Human Services' Area Agency on Aging, hereinafter referred to as "AAA," and
North Range Behavioral Health hereinafter referred to as "NRBH."
WITNESSETH:
WHEREAS, the "AAA" desires to continue to purchase peer counseling from "NRBH", and
WHEREAS, the "AAA" and "NRBH" are desirous of amending the original agreement of January
2, 1999,
NOW THEREFORE, it is mutually agreed:
1. The "AAA" will reimburse "NRBH" $32,737.00 for 1,800 contact units of peer counseling
for 90 persons.
2. All other terms and conditions of the original agreement shall remain in effect unless
otherwise stated in this Amendment.
3. This Amendment covers the period from January 2, 1999 to December 31, 1999.
