RE: APPROVE AMENDMENT TO AGREEMENT FOR PROFESSIONAL
CONSULTATION/TRAINING FOR WELD COUNTY JUVENILE DIVERSION PROGRAM
AND AUTHORIZE CHAIRMAN TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with an Amendment to Agreement for
Professional Consultation/Training between the County of Weld, State of Colorado, by and through
the Board of County Commissioners of Weld County, on behalf of the Juvenile Diversion Program,
and Scott Shannon, M.D., 1770 25th Avenue, Greeley, Colorado 80634, commencing
January 1, 1995, and ending December 31, 1995, with further terms and conditions being as stated
in said amendment, and
WHEREAS, after review, the Board deems it advisable to approve said amendment, a copy
of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Amendment to Agreement for Professional Consultation/Training
between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Juvenile Diversion Program, and Scott Shannon,
M.D., be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chairman be, and hereby is, authorized
to sign said amendment.
The above and foregoing Resolution was, on motion duly made and seconded, adopted by
the following vote on the 13th day of February, A.D., 1995, nunc pro tunc January 1, 1995.

AMENDMENT TO AGREEMENT FOR PROFESSIONAL CONSULTATION/TRAINING
FOR WELD COUNTY JUVENILE DIVERSION PROGRAM
This Amendment to Agreement is made and entered into thisl3th day of February, 1995,
by and between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of the County of Weld, by and on behalf of the Weld County Juvenile Diversion
Program, whose address is 915 Tenth Street, Greeley, Colorado 80631, hereinafter referred to as
"County," and Scott Shannon, M.D., whose address is 1770 25th Avenue, Greeley, Colorado
80634, hereinafter referred to as "Physician."
WITNESSETH:
WHEREAS, on January 12, 1994, the parties hereto entered into an Agreement entitled
"Agreement for Professional Consultation/Training for Weld County Juvenile Diversion
Program", wherein Physician agreed to provide consultation and -training to the counselors
involved in the Weld County Juvenile Diversion Program at the rate of $90.00 per hour, and
WHEREAS, said Agreement ended on December 31, 1994, and
WHEREAS, the parties now desire to amend said Agreement so as to extend the term
thereof from January 1, 1995, to and until December 31, 1995, and
WHEREAS, said amendment will also provide a cap for the amount of money to be spent
for such services.
NOW, THEREFORE, consideration of the mutual promises and covenants contained
herein, the parties hereto agree as follows:
1. That the Agreement entitled "Agreement for Professional Consultation/Training
for Weld County Juvenile Diversion Program" executed between and among the
parties on January 12, 1994, shall be extended for the year from January 1, 1995,
to and until December 31, 1995, according to the same terms and provisions as set
forth therein, with the following exceptions:
A. The cap for the amount of money to be spent by County for Dr. Shannon's
professional services consisting of "counselor supervision" shall be
$1,333.00 for the year 1995.
B. The cap for the amount of money to be spent by County for Dr. Shannon's
professional services consisting of "training Juvenile Diversion
counselors" shall be $1,170.00 for the year 1995.

SUBJECT: Request for Amendment to Renew Contract with Scott
Shannon for Calendar year 1995
The Juvenile Diversion Program is requesting to renew our
Professional Services Contract with Dr. Scott Shannon for the
period of January 1, 1995 through December 31, 1995.
Dr. Shannon's hourly rate will remain the same as in 1994: e Services (Counselor dSupervision9 #01-1125-c6350) r

Dr. Shannon will also be involved in training the Juvenile
Diversion Counselors. The cap for this expense will be $1170.00 to
be charged to Travel and Meetings (#01-1125-6370).
I am attaching a copy of the 1994 contract with Dr. Shannon for
your use in preparation of the amendment for the 1995 contract.
(NOTE: all terms will remain the same as for 1994)
