Home My WebLink About910556.tiff RESOLUTION RE: APPROVE AGREEMENT TO PROVIDE ADMINISTRATIVE SERVICES FOR COLORADO OLD AGE PENSIONERS DENTAL PROGRAM BETWEEN WELD COUNTY DEPARTMENT OF SOCIAL SERVICES AND STATE DEPARTMENT OF SOCIAL SERVICES AND AUTHORIZE CHAIRMAN TO SIGN WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to Colorado statute and the Weld County Home Rule Charter, is vested with the authority of administering the affairs of Weld County, Colorado, and WHEREAS, the Board has been presented with an Agreement to Provide Administrative Services for Colorado Old Age Pensioners Dental Program between Weld County Department of Social Services and the State Department of Social Services, with appropriations becoming available July 1, 1991, with the further terms and conditions being as stated in said Agreement, and WHEREAS, after review, the Board deems it advisable to approve said Agreement, a copy of which is attached hereto and incorporated herein by reference. NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld County, Colorado, that the Agreement to Provide Administrative Services for Colorado Old Age Pensioners Dental Program between Weld County Department of Social Services and the State Department of Social Services be, and hereby is, approved. BE IT FURTHER RESOLVED by the Board that the Chairman be, and hereby is, authorized to sign said Agreement. Kirby/Z^ H. e� ��W. i 910556 a 1S0013 �� Region 2-W June 1, 1991 AGREEMENT TO PROVIDE ADMINISTRATIVE SERVICES FOR COLORADO OLD AGE PENSIONERS DENTAL PROGRAM The State legislature has appropriated $396,238.00 for care and administration for the Colorado Old Age Pensioners Dental Program for fiscal year 1991/92. The appropriations become available July 1, 1991. The State requires assistance in areas of the State to receive and process applications and to determine eligibility of recipients. The implementing agency shall provide the following services: 1) Receive applications from prospective patients. 2) Determine eligibility of. applicants. 3) Assist old age pensioners in making dental appointments with dentists who are participating in the program. 4) Assist old age pensioners in obtaining transportation to the dental office when necessary. 5) Determine the amount of co-payment to be made by patient. 6) Assist old age pensioners in obtaining funds for the co-payment when indicated. 7) Maintain records of patients, services provided and funds encumbered and spent. 8) Provide regular reports of persons served, services rendered and funds encumbered and spent. al ir..r `" AGREEMENT TO PROVIDE ADMINISTRATIVE SERVICES page 2 Colorado Old age Pensioners Dental Program The State will, in consideration of said services, pay to the Region a sum not to exceed One Thousand Dollars ($1,000 ) in the following manner: 1) Upon receipt of a signed monthly (no less than quarterly) statement submitted in duplicate, requesting reimbursement for services; and conditioned upon affirmation by the Dental Health Section that such services were performed. 2) The Region shall be paid $22.00 per processed form. The form shall be considered processed upon completion of the patient treatment and receipt, by the Colorado Department of Health, Dental Health Section, of the white and green copies of the authorization for service form signed by the Dental Committee member, dated and signed by the dentist. In the event the patient is required to be referred to additional dentists for consulation and/or treatment, only the original form processing will be considered for payment. 3) To be considered for payment, billings for reimbursement must be received within 60 days after the period for which payment is being requested. 4) In January, each year, spending patterns are reviewed in order that all appropriated dollars may be wisely expended. It is agreed that some portion of the above allocation may be withdrawn if spending patterns indicate that there will be money unused during the State fiscal year.
