RE: APPROVE FAMILY PLANNING CONTRACT CHANGE LETTER 01 BETWEEN HEALTH
DEPARTMENT AND COLORADO DEPARTMENT OF HEALTH AND AUTHORIZE CHAIRMAN TO
SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado,
pursuant to Colorado statute and the Weld County Home Rule Charter, is vested
with the authority of administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Family Planning Contract
Change Letter 01 between the County of Weld, State of Colorado, by and through
the Board of County Commissioners of Weld County, on behalf of the Health
Department, and the Colorado Department of Health, for the contract year
beginning July 1, 1993, and ending June 30, 1994, with further terms and
conditions being as stated in said letter, and
WHEREAS, after review, the Board deems it advisable to approve said change
letter, a copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Family Planning Contract Change Letter 01 between the
County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Health Department, and the
Colorado Department of Health be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chairman be, and hereby is,
authorized to sign said change letter.
The above and foregoing Resolution was, on motion duly made and seconded,
adopted by the following Adteh

Pursuant to Paragraph 7 of the Family Planning Contract, Contract # FPP940088,
[as amended] , covering the period July 1, 1993 to June 30 1994, the Colorado
Department of Health hereby notifies Weld County Health Department that the
maximum amount of reimbursement from the Family Planning Program is hereby
decreased by Sixteen thousand eight hundred seventy-five dollars ($16.875) to
a new total of One Hundred fifty-five thousand Two Hundred Ninety-eight Dollars
($155,298) (Page 3, Paragraph 6) . These funds are all Waxman dollars.
The number of comprehensive family planning patients to be served is to remain
unchanged; Colposcopy, biopsy and/or pap smear screening services are to be
decreased by 75 to a new total of 100 ; Cryosurgery services and/or pap smear
screening services are to remain unchanged.
This contract change letter is intended to be effective May 6, 1994 , but in no
event shall it be deemed valid until it is approved by the State Controller or
his designee.
Please sign and return all copies of this notification to the Family Planning
Program. A fully executed copy will be returned to you.

Enclosed for Board approval is a contract change letter from the Family
Planning Program at the Colorado Department of Health (CDH) .
The change in the contract will decrease our funding for the time period
July 1, 1993 through June 30, 1994 by $16,875. Last year, WCHD performed a
high number of colposcopy procedures due to abnormal pap smears on Family
Planning patients. This year, the number of these procedures has decreased.
CDH evaluates each year the services the local health departments are
providing and reallocates the funding if necessary. They decrease funding in
programs of less need to assist programs which are exhibiting an increased
need.
This decrease should not affect us adversely as revenue for this account was
conservatively estimated (in preparation for such possible decreases) in the
1994 budget. I recommend your approval of this change.
