RE: APPROVE CONTRACT RENEWAL LETTER FOR FAMILY PLANNING PROGRAM AND AUTHORIZE
CHAIRMAN TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado,
pursuant to Colorado statute and the Weld County Home Rule Charter, is vested
with the authority of administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Contract Renewal Letter for
the Family Planning Program between the County of Weld, State of Colorado, by and
through the Board of County Commissioners of Weld County, on behalf of the Weld
County Health Department, and the Colorado Department of Health, commencing July
1, 1994, and ending June 30, 1995, with further terms and conditions being as
stated in said letter, and
WHEREAS, after review, the Board deems it advisable to approve said letter,
a copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Contract Renewal Letter for the Family Planning
Program between the County of Weld, State of Colorado, by and through the Board
of County Commissioners of Weld County, on behalf of the Weld County Health
Department, and the Colorado Department of Health be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chairman be, and hereby is,
authorized to sign said letter.
The above and foregoing Resolution was, on motion duly made and seconded,
adopted by the following vote on the 20th day of July, A.D. , 1994, nunc pro tunc

CONTRACT RENEWAL LETTER Executive Director

The Colorado Department of Health hereby notifies Weld County Health Department
that pursuant to paragraph 10 of contract number FPP940088, the state will
exercise its option to renew this contract for an additional one year period
beginning July 1, 1994 through June 30, 1995. Paragraph 6 of the contract is
deleted and replaced with the following (Paragraphs 6a through 6g remain
unchanged) :
The State will pay Eighty-five thousand seven hundred eighty-four dollars
(S85.784) ($63,566 Title X dollars and $22,218 State dollars) to provide
contraceptive services to approximately 1900 family planning patients; and Two
thousand six hundred twenty-five dollars ($2.625) to provide cryosurgery and/or
pap smear screening services to _1L patients. In consideration of said services
by the Contractor and conditioned upon affirmation by the Family Planning Program
that services were rendered in accordance with this contract, the state will
cause to be paid to the Contractor a sum not to exceed Eighty-eight thousand
four hundred nine dollars ($88.409) .
This contract renewal letter is intended to be effective July 1, 1994 , but in
no event shall it be deemed valid until it is approved by the State Controller
or his designee.
Please sign and return all copies of this notification. A fully executed copy
will be returned to you.

Enclosed for Board approval is a contract between the Weld County Health
Department and the Colorado Department of Health for our Family Planning
Program for the period July 1, 1994 through June 30, 1995.
The Health Department will be reimbursed $85,784 to provide contraceptive
services to 1,900 comprehensive Family Planning clients and $2,625 to provide
cryosurgery for 35 clients. These amounts equal a total contract amount of
$88,409 which is $ 66,913 less than last year's contract. This decrease in
funding is due to the following:
1. Last year's contract provided $22,500 for colposcopy and biopsy
and/or pap screening services to 175 patients. This year, these
funds will be included in a Cancer Control Project Contract which we
have not yet received.
2. Funds to provide chlamydia testing to 415 clients are now included
in the Chlamydia Contract.
3. Funding for cryosurgery has been decreased by $1,875 to serve 35
clients instead of the 45 funded last year.
The amount of allocated funding is based on a formula. A decrease in the
number of clients served caused our overall funding to decrease.
I recommend your approval of this contract.
