RE: APPROVE PREVENTIVE HEALTH AND HEALTH SERVICES BLOCK GRANT APPLICATION TO
COLORADO DEPARTMENT OF HEALTH AND AUTHORIZE CHAIRMAN TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado,
pursuant to Colorado statute and the Weld County Home Rule Charter, is vested
with the authority of administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Preventive Health and Health
Services Block Grant Application to Colorado Department of Health, commencing
August 2, 1993, and ending September 30, 1994, with the further terms and
conditions being as stated in said application, and
WHEREAS, after review, the Board deems it advisable to approve said
application, a copy of which is attached hereto and incorporated herein by
reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Preventive Health and Health Services Block Grant
Application to Colorado Department of Health be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chairman be, and hereby is,
authorized to sign said application.
The above and foregoing Resolution was, on motion duly made and seconded,
adopted by the following vote on the 26th day of May, A.D. , 1993.

PROJECT SUMMARY
In 300 words or less, provide a brief summary of what the project is, how it will be
implemented and the anticipated results. (Attach a separate sheet if necessary) .
In 1990, using the Healthy People 2000 document as a foundation, the Weld County Health
Department (WCHD) undertook the process of developing county specific health goals for the
year 2000. A preventive medicine resident from the University of Colorado's Health Sciences
Center compiled baseline data on The County health indices. The baseline data and national
objectives were used to develop objectives in the areas of maternal and child health, family
planning, communicable disease, injury prevention, chronic disease prevention and
environmental health. The resulting document has become the framework for WCHD program
planning.
Two needs have clearly emerged after two years of working with the current document. First,
there are notable gaps in the measurable data pertaining to the objectives for Environmental
Health necessitating an environmental health assessment in Weld County. Second, even though
a number of efforts towards developing an implementation plan have resulted in the beginnings
of a plan, finalization of the plan always takes a back seat to the more pressing issues of
day to day departmental operation. One staff member dedicated to this project will allow the
Weld County Health Department to complete our strategic planning efforts.
A full time health planner will be hired. The health planner will work with a community
advisory board, the Weld County Advisory Board of Health and Health Department staff to
complete a community environmental health assessment establishing a baseline of community
perception of environmental health problems. The health planner will use the APEX planning
model to develop an implementation plan addressing the concerns identified by the assessment
process and the health objectives outlined in the document Weld County Health Objectives for
the Year 2000.
The anticipated outcome of this project will be the development of an action plan guiding the
Weld County Health Department towards achieving the Health Objectives for the Year 2000.

PROGRAM PLANNING ELEMENTS
Please answer the following questions:
1. Problem Statement
What is the need? What is the evidence that the stated problem(s) is a high priority
for the applicant? Please include any relevant materials or summaries as attachments.
Due to lack of resources, the program planning process for the Weld County Health
Department is lacking a comprehensive environmental needs assessment. Consequentially, the
Department lacks a focus in the subsequent goals, objectives, methodology, implementation and
evaluation. The department lacks a comprehensive assessment of the environmental needs of
Weld County in terms of social and epidemiological diagnoses. There is no comprehensive
assessment of the perceived environmental and community health needs County. In addition,
there is no epidemiological data that has been collected to comprehensively characterize the
environmental health status.
The environmental goals are based on traditional activities and may not accurately
reflect the real needs of the community. The Weld County 2000 Objectives for the environment
are not clearly defined or measurable.
The Department has conducted traditional activities and implemented generally accepted
methods for providing services. Other methods have not been explored, which in the light of
a comprehensive needs assessment, may be more effective or appropriate to the population of
Weld County.
Because of the lack of an understanding of the needs of Weld County, the Environmental
Protection Services Division has no established criteria by which to base decisions on
resource allocation and program direction. There is no established baseline to assess the
effectiveness of the Division in meeting the mission statement. The Weld County 2000
Objectives for the Department are clearly defined for the community health issues in 1991,
but have not been revisited to assess progress. The Department has not evaluated existing
activities to determine effectiveness and appropriateness in light of a comprehensive needs
assessment.
The stated problems are a high priority for Weld County as evidenced by the fact that
they include increasing demands on existing staff and limited scope of existing assessment
attempts. Increasing demands on staff time for an increasing number of issues and concerns
has exhausted resources which should be dedicated to community needs assessment.
Administrative staff has had to devote their time to handling issues and concerns on a case
by case basis without a comprehensive assessment to aid in prioritization. Current
indicators and past assessments have been limited in scope and may not be indicative of the
needs of the County and may exclude or overlook other real concerns or issues. Local
assessments which have been performed have been program specific and limited specifically to
the Greeley area. Although these surveys have indicated local concern for air quality
issues, they are not generalizable to all of Weld County and may not be indicative of the
concerns of rural Weld County. The staff spends 30% of its time investigating complaints in
nine program areas. However, approximately 90% of those complaints ultimately concern issues
that are not directly addressed in the current program plan. If these complaints serve as
a viable social or epidemiological indicator of environmental problems in Weld County, one
can deduct that current program design is lacking in many areas of concern: odors, fugitive
dust, nuisance waste issues, etc.

2. Goals and Objectives
How will this project help Colorado reach the Year 2000 objectives?
Goal: To develop a comprehensive environmental and community health program plan for
Weld County including a performance of a comprehensive needs assessment; identification of
goals and objectives; exploration of alternative methods; modification of current
implementation plan; and development of impact and outcome evaluation strategies.
Objectives. The project objectives are as follows:
1. A comprehensive environmental health needs assessment for Weld County will be conducted
by March 23, 1994.
2. One goal for each environmental and community health program will be developed
reflecting the needs of Weld County by April 8, 1994.
3. One objective for each environmental and community health program goal will be
determined by April 14, 1994.
4. Existing and alternative methodologies for program delivery will be evaluated and
explored and program methodologies will be determined which are consistent with the
needs assessment, goals and objectives by May 20, 1994.
5. Implementation plans will be designed to meet the program objectives by June 7, 1994.
6. A comprehensive evaluation plan will be developed by September 20, 1994 to evaluate the
current status of health in Weld County and monitor program effectiveness in terms of
impact and outcome in the future.

3. MODEL USE
Explain what model will be used and why it was selected.
In 1991, the staff of the Weld County Health Department prioritized the health
objectives identified in Healthy People 2000, selecting the objectives most meaningful
for our population. Objectives relating to maternal and child health, family planning,
communicable disease, injury prevention, chronic disease prevention and environmental
health promotion were adopted intact or modified to fit the health risks and size of
our community. These objectives have become the framework for program planning. With
the exception of many of the environmental health objectives, baseline measurement has
been established for many of the objectives, what needs to follow is the development
of implementation and evaluation plan.
To make use of the groundwork already done during the development of this document, the
planner would lead the WCHD executive staff through a modified APEX process to develop
the implementation plan. Such a strategy would allow us to draw on the experience of
other organized health departments in Colorado who have already begun to implement APEX
through previous grant awards.

4. TARGET POPULATION
What is the target population relative to the Healthy People 2000 objectives? What
percentage of the target population will be reached?
It is believed that each resident of Weld County will be impacted in some manner by the
implementation of a long range strategic plan. By developing an implementation and
evaluation plan, limited resources will be better focused in areas of greatest need and
the success of such efforts will be more readily measured. WOW, currently provides
family planning, chronic disease prevention and maternal and child health services to
over XXX clients each year. It is estimated that an additional XXX Weld County
residents are impacted through environmental protection programs including food safety,
water and air quality monitoring and solid waste management. Weld County is bordered
by several rural counties which have limited access to health care, WCHD provides
additional services to these communities.

6. BARRIERS/OBSTACLES
What problems, if any, must be solved to complete the proposed planning process?
The most significant barrier Weld County faces to completing the planning process is
that we simply lack the time as an executive staff to develop the implementation plan
begun previously. By hiring a full time staff member responsible for leading the
process we will be able to overcome this barrier.

7. PROJECT OUTCOME
Will the project become self-supporting, transferred or terminated? How and by whom?
How will this planning process affect future activities in your area related to
reaching the Year 2000 objectives?
Upon completion of the funding year the project will terminate with the development of
a document to guide program prioritization, implementation and evaluation in Weld
County. Project monitoring and evaluation will be the responsibility of the executive
staff of the Weld County Health Department.
