RE: APPROVE TUBERCULOSIS PROGRAM EXPANSION PROPOSAL TO THE COLORADO DEPARTMENT
OF HEALTH
WHEREAS, the Board of County Commissioners of Weld County, Colorado,
pursuant to Colorado statute and the Weld County Home Rule Charter, is vested
with the authority of administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Tuberculosis Program Expansion
Proposal to Colorado Department of Health, with the terms and conditions being
as stated in said proposal, and
WHEREAS, after review, the Board deems it advisable to approve said
proposal, a copy of which is attached hereto and incorporated herein by
reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Tuberculosis Program Expansion Proposal to the
Colorado Department of Health be, and hereby is, approved.
The above and foregoing Resolution was, on motion duly made and seconded,
adopted by the following vote on the 2nd day of November, A.D. , 1992.

OBJECTIVE: Expand TB surveillance, prevention, and containment in order to reach
groups at risk for TB.
DESCRIPTION: Weld County is mainly a rural area covering 4000 square miles.
There are 2,500,000 acres used for agriculture, with 96% devoted to farming and
raising livestock. Six point nineteen percent of the total employment is in
agriculture. There is a large migrant population present to support this
agricultural industry. With the migrant population, there is a housing shortage
and poor access to health care, both increasing the risk of TB transmission.*
In 1989, 14% of the population of Weld County lived in areas without a health
care provider. Fourteen percent of the residents live in poverty. The average
per capita income in 1989 was $14,230 in Weld County compared to $17,682 in !!
Colorado and $17,592 in the United States. Weld County's ethnic distribution
consists of 77% white, 21% Hispanic, and 2% Black/other.*
Island Grove Treatment Center is a public treatment facility for persons who
abuse substances and serves clients from a multi county region in Northeastern
Colorado. This agency has previously requested assistance from the Health
Department to screen and treat their high risk clients.
Monfort Inc. is a meat processing plant employing over 4000 employees, the
largest employer in Weld County. A majority of these employees are Hispanic and
a large portion are migrant workers in unskilled labor positions. Monfort has
an active acute care health center. The Medical Director of Monfort has agreed
to work with Weld County Health Department in implementing a program to diagnose
and treat employees infected with TB.
The Restitution Center is a minimum security correctional facility located in
Greeley. The Center currently does no TB surveillance except for employees.
There are two Community Health Centers in the county serving a large number of
the Hispanic and low income persons. These centers do limited TB surveillance and
control.
Weld County Health Department currently provides a limited amount of TB
surveillance and outreach. This current activity is targeted to high risk groups
in the jail, low socioeconomic prenatal clients, and nursing home and day care
employees. The Health Department is responsible for evaluating and prescribing
TB therapy for the entire county. Health Department TB staff consist of a part
time bilingual outreach worker and a part time nurse coordinator. Outreach to
identified high risk groups is restricted due to limited funding. Additional
resources would allow greater outreach to high risk groups and thus more
surveillance and containment. This proposal requests increased funding to hire
an additional full time bilingual outreach worker and a full time secretary in
order to direct efforts to the identified high risk populations

TARGET POPULATION #1: Persons with a history of substance abuse
Action Steps: 1) implement screening on site at the
drug rehabilitation facility.
2) provide INH preventive therapy to
prevent clinical disease among substance abusers, to include
injection drug users and alcoholics.
TARGET POPULATION #2: Migrant and seasonal workers, racial and ethnic minority
groups
Action Steps: 1) implement screening on site at the Greeley Monfort
beef plant
2) provide INH preventive therapy to prevent clinical
disease among migrant/seasonal workers and racial and ethnic
minority groups
3) investigate the feasibility of on site DOT utilizing
the Monfort employee health staff and facility.
4) coordinate with the two Community Health Centers to
promote screening of migrant farm workers.
TARGET POPULATION #3: persons in correctional facilities
Action Step: provide outreach and education to the Restitution
Center.
EVALUATION: Increase the number of at risk persons placed on INH preventive
therapy and increase the number who complete preventive treatment.

SubjectTuberculosis Program Expansion Proposal
Enclosed for Board approval is a proposal to Colorado Department of Health for
expansion of the Weld County Health Department's Tuberculosis (TB) Program. The
state may receive additional TB funds and requested local health departments
proposals for these funds.
The Weld County Health Department is requesting $47,753 in additional annual
funds to expand TB surveillance to high-risk groups in Weld County. Six
agencies, Island Grove Treatment Center, Monfort Inc. , Restitution Center,
Sunrise Community Health Center, and Plan de Salud del Valle, will work with the
Health Department to reach high-risk clients within their agency clientele.
Program approval and funding will be determined only if federal funds are
allocated to the Colorado Department of Health.
