Griego Telephone #: 352-1551 Ext. 6200 Federal Objective/s Grant Complies With: X a: To improve the coordination of existing private and public assistance resources to better serve low-income communities; X b: To assist low-income communities to identify potential sponsors of child nutrition programs and initiate programs in unserved and under-served areas; and X c: To develop innovative approaches at State and local levels to meet nutritional needs of low-income individuals. Applicant Program Description: The Weld Food Bank, a not-for profit agency, accepts food donations from various local producers and suppliers. Other sources of obtaining food are food drives, second Harvest National Food Bank Network, TEFAP, and purchasing food. In operating an Emergency Food Box program for over six years, we have noticed some low income hungry persons who seem to "slip through the cracks" and are not being served by the food network. By working with the Coordinated Food Box Committee, which is made up of 25 local agency representatives, a Supplemental Mini Box was designed. This concept has worked quite well during the past year. Based on the amount requested, Weld Food Bank would serve 1,200 persons, or approximately 100 per month. The average cost per person is $3.00 per month. Included in the Mini Box is a protein item such as dairy products, soup and macaroni and cheese dinner. Extra food items are included such as fresh eggs, vegetables, bread, sweets, as well as other food items based on supply. All referrals are made through Weld Information and Referral Service and the Salvation Army. The goal of the Community Food and Nutrition Program is to feed hungry people and prevent the waste of some sporadic large perishable donations. Weld County Department of Social Services will be responsible for the administration of the program. Signature of Authorized Representative: cs 1,,.e s _ 064.3 9 :6OSfa,ii. For the past three years the County has been awarded funds through this program and has contracted with Weld Food Bank for implementation of the program. This arrangement has proved to be quite workable and is being recommended once again this year. The amount available through the Community Food and Nutrition program this year is $3,650. We are therefore recommending that the County approve the application as prepared. Upon Board approval, the application will be forwarded to the Department of Local Affairs for processing.
