RE: APPROVE PURCHASE OF SERVICE AGREEMENTS BETWEEN THE AREA AGENCY ON AGING
AND VARIOUS LOCAL AGENCIES AND AUTHORIZE CHAIRMAN TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado,
pursuant to Colorado statute and the Weld County Home Rule Charter, is vested
with the authority of administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with seven Purchase of Service
Agreements between the Weld County Area Agency on Aging and the following local
agencies:
Meals on Wheels
Catholic Community Services Northern (two agreements)
RVNA Home Care Services
Eldergarden Adult Day Program
Charles J. Connell, Attorney at Law
Weld Mental Health Center
WHEREAS, the term of said agreements is January 2, 1993, through December
31, 1993, with the further terms and conditions being as stated in said
agreements, and
WHEREAS, after review, the Board deems it advisable to approve said
agreements, copies of which are attached hereto and incorporated herein by
reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the seven Purchase of Service Agreements between the Weld
County Area Agency on Aging and the above listed local agencies be, and hereby
are, approved.
BE IT FURTHER RESOLVED by the Board that the Chairman be, and hereby is,
authorized to sign said agreements.
The above and foregoing Resolution was, on motion duly made and seconded,
adopted by the following vote on the 6th day of January, A.D. , 1993, nunc pro
tunc January 2, 1993.

PURCHASE OF SERVICES AGREEMENT AMENDMENT
THIS AGREEMENT, made and entered into this 2nd day of January, 1993,
by and between the Weld County Division of Human Resources' Area Agency on Aging,
hereinafter referred to as "AAA," and Catholic Community Services Northern,
hereinafter referred to as "CCSN."
WITNESSETH:
WHEREAS, the "AAA" desires to continue to purchase Hispanic elderly
outreach from "CCSN", and
WHEREAS, the "AAA" and "CCSN" are desirous of amending the original
agreement of January 2, 1992,
NOW THEREFORE, it is mutually agreed:
1. The "AAA" will reimburse "CCSN" $25,653.00 for 3,700 units of
outreach for 280 isolated and homebound Hispanic elders.
2. All other terms and conditions of the original agreement shall
remain in effect unless otherwise stated in this Amendment.
3. The terms of this agreement amendment extend from January 2, 1993
through December 31, 1993.

PURCHASE OF SERVICES AGREEMENT AMENDMENT
THIS AGREEMENT, made and entered into this 2nd day of January, 1993,
by and between the Weld County Division of Human Resources' Area Agency on Aging,
hereinafter referred to as "AAA," and Catholic Community Services Northern,
hereinafter referred to as "CCSN."
WITNESSETH:
WHEREAS, the "AAA" desires to continue to purchase respite care from
"CCSN", and
WHEREAS, the "AAA" and "CCSN" are desirous of amending the original
agreement of January 2, 1992,
NOW THEREFORE, it is mutually agreed:
1. The "AAA" will reimburse "CCSN" $7,931.00 for 1,400 units of home
respite care for 26 families.
2. All other terms and conditions of the original agreement shall
remain in effect unless otherwise stated in this Amendment.
3. The terms of this agreement amendment extend from January 2, 1993
through December 31, 1993.

PURCHASE OF SERVICES AGREEMENT AMENDMENT
THIS AGREEMENT, made and entered into this 2nd day of January, 1993,
by and between the Weld County Division of Human Resources' Area Agency on Aging,
hereinafter referred to as "AAA," and Meals on Wheels hereinafter referred to as
"MOW."
WITNESSETH:
WHEREAS, the "AAA" desires to continue to purchase home delivered meals
from "MOW", and
WHEREAS, the "AAA" and "MOW" are desirious of amending the original
agreement of January 2, 1992,
NOW THEREFORE, it is mutually agreed:
1. The "AAA" will reimburse "MOW" $28,378.00 for 25,400 home deliverd
meals for 120 persons.
2. All other terms and conditions of the original agreement shall
remain in effect unless otherwise stated in this Amendment.
3. The terms of this Amendment shall extend from January 2, 1993
through December 31, 1993.

PURCHASE OF SERVICES AGREEMENT AMENDMENT
THIS AGREEMENT, made and entered into this 2nd day of January, 1993,
by and between the Weld County Division of Human Resources' Area Agency on Aging,
hereinafter referred to as "AAA," and RVNA Home Care Services, Inc. , hereinafter
referred to as "RVNA."
WITNESSETH:
WHEREAS, the "AAA" desires to continue to purchase home health/homemaker
aide from "RVNA", and
WHEREAS, the "AAA" and "RVNA" are desirious of amending the original
agreement of January 2, 1992,
NOW THEREFORE, it is mutually agreed:
1. The "AAA" will reimburse "RVNA" $21,390.00 for 2,770 units of home
health/homemaker aide for 55 persons.
2. All other terms and conditions of the original agreement shall
remain in effect unless otherwise stated in this Amendment.
3. The terms of this Amendment shall extend from January 2, 1993
through December 31, 1993.

PURCHASE OF SERVICES AGREEMENT AMENDMENT
THIS AGREEMENT, made and entered into this 2nd day of January, 1993,
by and between the Weld County Division of Human Resources' Area Agency on Aging,
hereinafter referred to as "AAA," and Charles J. Connell, Attorney at Law,
hereinafter referred to as "Connell."
WITNESSETH:
WHEREAS, the "AAA" desires to continue to purchase attorney consultation
and supervision of the Area Agency on Aging Senior Legal Assistance Program.
WHEREAS, the "AAA" and "Connell" are desirious of amending the original
agreement of January 2, 1992,
NOW THEREFORE, it is mutually agreed:
1. The "AAA" will reimburse "Connell" $32.50 per hour for consultaion
time and $42.50 per hour for court time not to exceed $5000.00.
2. All other terms and conditions of the original agreement shall
remain in effect unless otherwise stated in this Amendment.
3. This Amendment covers the period from January 2, 1993 to December
31, 1993.

PURCHASE OF SERVICES AGREEMENT AMENDMENT
THIS AGREEMENT, made and entered into this 2nd day of January, 1993,
by and between the Weld County Division of Human Resources' Area Agency on Aging,
hereinafter referred to as "AAA," and Weld Mental Health, Inc. hereinafter
referred to as "WMH."
WITNESSETH:
WHEREAS, the "AAA" desires to continue to purchase peer counseling from
"WMH", and
WHEREAS, the "AAA" and "WMH" are desirious of amending the original
agreement of January 2, 1992,
NOW THEREFORE, it is mutually agreed:
1. The "AAA" will reimburse "WMH" $23,737.00 for 4,650 contact units
of peer counseling for 285 persons.
2. All other terms and conditions of the original agreement shall
remain in effect unless otherwise stated in this Amendment.
3. This Amendment covers the period from January 2, 1993 to December
31, 1993.

PURCHASE OF SERVICES AGREEMENT AMENDMENT
THIS AGREEMENT, made and entered into this 2nd day of January, 1993,
by and between the Weld County Division of Human Resources' Area Agency on Aging,
hereinafter referred to as "AAA," and Eldergarden Adult Day Program, hereinafter
referred to as "ELDERGARDEN."
WITNESSETH:
WHEREAS, the "AAA" desires to continue to purchase adult day care from
"Eldergarden", and
WHEREAS, the "AAA" and "Eldergarden" are desirious of amending the original
agreement of January 2, 1992,
NOW THEREFORE, it is mutually agreed:
1. The "AAA" will reimburse "Eldergarden" $13,950.00 for 16,500 units
of adult day care for 85 persons.
2. All other terms and conditions of the original agreement shall
remain in effect unless otherwise stated in this Amendment.
3. The terms of this Amendment shall extend from January 2, 1993
through December 31, 1993.

Enclosed for board approval are Purchase of Service Agreements between the
Area Agency on Aging and the following local agencies to provide services
as specified in the Area Plan for 1993;
1. Meals on Wheels - Purchase 25,400 meals for 120 homebound elderly of
Weld County. Total amount allocated is $28,378.00.
2. Catholic Community Services Northern - Purchase 3,700 units of
outreach to 280 isolated and homebound Hispanic elders. Total
amount allocated is $25,653.00.
3. Catholic Community services Northern - Purchase 1,400 units of home
respite care for 26 families. Total amount allocated is $7,931.00.
4. RVNA Home Care Services - Provide 2,770 units of home
health/homemaker aide for 55 persons. Total amount allocated is
$21,390.00.
5. Eldergarden Adult Day Program - Provide 16,500 units of adult day
care to 85 persons. Total amount allocated is $13,950.00.
6. Charles J. Connell Attorney at Law - Provide attorney supervision of
the Area Agency Senior Citizen's Law Project at $32.50 per hour for
consultation time and $42.50 per hour for court appearance time.
7. Weld Mental Health - Provide 4,650 hours of peer counseling for 285
homebound elderly. Total amount allocated is $23,737.00.
