RE: APPROVE AMENDMENT TO AGREEMENT FOR SHERIFF'S OFFICE JAIL MEDICAL UNIT
SERVICE WITH PREFERRED CORRECTIONAL MEDICAL TEAM, INC. , AND AUTHORIZE
CHAIRMAN TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado,
pursuant to Colorado statute and the Weld County Home Rule Charter, is vested
with the authority of administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with an amendment to the Agreement
for Weld County Sheriff's Office Jail Medical Unit Service between the County of
Weld, State of Colorado, by and through the Board of County Commissioners of Weld
County, on behalf of the Weld County Sheriff's Office, and Preferred Correctional
Medical Team, Inc. , for an alteration of the monthly payment amount to
$39,020.00, said agreement commencing August 1, 1994, and ending December 31,
1994, with further terms and conditions being as stated in said amendment, and
WHEREAS, after review, the Board deems it advisable to approve said
amendment, a copy of which is attached hereto and incorporated herein by
reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the amendment to the Agreement for Weld County Sheriff's
Office Jail Medical Unit Service between the County of Weld, State of Colorado,
by and through the Board of County Commissioners of Weld County, on behalf of the
Weld County Sheriff's Office, and Preferred Correctional Medical Team, Inc. , be,
and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chairman be, and hereby is,
authorized to sign said amendment.
The above and foregoing Resolution was, on motion duly made and seconded,
adopted by the following vote on the 11th day of July, A.D. , 1994.

AMENDMENT TO AGREEMENT FOR WELD COUNTY SHERIFF' S
OFFICE JAIL MEDICAL UNIT SERVICE
This Amendment, dated for reference purposes only the II -h
day of July, 1994, is made and entered into by and between the
Board of County Commissioners of Weld County, Colorado ( "County" )
and Preferred Correctional Medical Team, Inc. ( "Provider" ) . It is
hereby attached to and shall become a part of the Agreement by and
between the County and Provider which was executed September 11,
1988 .
WITNESSETH:
WHEREAS, the County and Provider entered into a Medical Unit
Service Agreement ( "Agreement" ) on September 11, 1988; and
WHEREAS, said Agreement was renewed and extended for the
period from January 1 through December 31, 1994, by Amendment dated
December 20, 1993; and
WHEREAS, said Amendment set the monthly pay amount at
$32 ,000 .00 per the period of January 1 through December 31, 1994 ;
and
WHEREAS, the parties hereto agree that there is a need for an
increase in said monthly payment amount for the period August 1
through December 31, 1994 , due to increased numbers of inmates
currently residing at the Weld County Jail .
NOW, THEREFORE, for good and valuable consideration, the
receipt and sufficiency of which are hereby acknowledged, the
County and Provider agree to the following:
1 . That the monthly payment amount of $32, 000 . 00 is hereby
adjusted to $39,020 . 00 for the period from August 1, 1994, to
December 31, 1994 .
2 . That the Agreement, as amended December 20, 1993, and as
amended hereby, shall remain in full force and effect through
December 31, 1994 .
IN WITNESSETH WHEREOF, the parties hereto have executive this
amendment this //fh day of July, 1994 .
