HomeMy WebLinkAbout931063.tiff RESOLUTION RE: APPROVE PURCHASE OF SERVICE AGREEMENT WITH THE GREELEY DREAM TEAM, INC. AND AUTHORIZE CHAIRMAN TO SIGN WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to Colorado statute and the Weld County Home Rule Charter, is vested with the authority of administering the affairs of Weld County, Colorado, and WHEREAS, the Board has been presented with a Purchase of Service Agreement between the County of Weld, State of Colorado by and through the Board of County Commissioners of Weld County, on behalf of the Weld County Division of Human Resources' Family Educational Network of Weld County, Head Start Program, and The Greeley Dream Team, Inc. , commencing July 1, 1993, and ending June 30, 1994, with further terms and conditions being as stated in said agreement, and WHEREAS, after review, the Board deems it advisable to approve said agreement, a copy of which is attached hereto and incorporated herein by reference. NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld County, Colorado, that the Purchase of Service Agreement between the County of Weld, State of Colorado by and through the Board of County Commissioners of Weld County, on behalf of the Weld County Division of Human Resources' Family Educational Network of Weld County, Head Start Program, and The Greeley Dream Team, Inc. be, and hereby is, approved. BE IT FURTHER RESOLVED by the Board that the Chairman be, and hereby is, authorized to sign said agreement. The above and foregoing Resolution was, on motion duly made and seconded, adopted by the following vote on the 13th day of October, A.D. , 1993, nunc pro tunc July 1, 1993. Kir yer O 931063 U Roo 51 ,1 ' : ;i/ PURCHASE OF SERVICE AGREEMENT THIS AGREEMENT, made and entered into this /g 13 day of (% wy' 1993, by and between the County of Weld, State of Colorado by and through the Board of County Commissioners of Weld County, on behalf of the Weld County Division of Human Resources' Family Educational Network of Weld County, Head Start Program, hereinafter referred to as "FENWC" and The Greeley Dream Team', Inc. , hereinafter referred to as "Dream Team." WITNESSETH WHEREAS, The Dream Team received an Even Start grant from the Colorado Department of Education for the Period of July 1, 1993 through June 30, 1997. The purpose of this grant is to promote family literacy to eligible participants (Department of Education, Chapter I criteria) within Greeley/Evans School District 6. WHEREAS, The Dream Team desires to utilize FENWC Service Coordinators to fulfill the objectives of the First and Foremost Even Start Project. WHEREAS, the parties desire to define their respective obligations and duties to each other. NOW, THEREFORE, in accordance with the mutual benefits, responsibilities and considerations set forth herein, the parties hereto covenant and agree as follows: 1. Recitals. The recitals listed above are incorporated herein by reference and shall be utilized in interpreting the Contract. 2. Term of Contract. The term of this Contract shall be from July 1, 1993 to June 30, 1994 and may be renewed each of the subsequent years of the grant if both parties agree to do so. 3. Obligations of FENWC. During the term hereof, FENWC agrees to provide the following services. a. Provide .50 FTE FENWC Coordination to be divided equally among the three project sites (East Memorial Elementary School, Island Grove Village Apartments & Centennial Elementary School) and personnel with recruiting Chapter I eligible participants and providing project services, including parenting education and training. b. Provide classroom space where available at each of the three target sites for project activities and parent participation. c. FENWC Service Coordinators will also assist in the articulation of curriculum at the preschool and elementary levels and training for staff at each of the levels. d. Maintain adequate records and files on project participants as outlined in the grant for usage with grant management and reporting. 4. Payment of Obligations. The Dream Team will reimburse the total sum of $10,000.00 which shall be broken down into twelve monthly payments of $833.33. FENWC will be responsible to submit a reimbursement form to the Dream Team by the 5th of each month to assure timely payment. 5. Progress Reports. FENWC shall submit semiannual reports to the Executive Director of the Dream Team. The reports shall provide information related to progress in carrying out the identified tasks. These reports shall be due by January 31, 1994, and July 31, 1994. 6. Financial and Accounting Obligations. The Dream Team shall fully comply with all fiscal, financial and record keeping obligations imposed by the terms of the grant, and as otherwise may be imposed by law. 7. Nondiscrimination. The Dream Team and FENWC assure compliance with Title VI of the Civil Rights Act of 1986, and that no person shall, on the grounds of race, creed, color, sex or national origin, be excluded from participation in, be denied the benefits of, or otherwise subjected to discrimination under the approved Agreement. 8. Termination of Agreement. This agreement may be amended or terminated upon thirty (30) days written notice to either party. This Agreement will be terminated immediately upon the loss of funding to the Dream Team.
