RE: APPROVE PURCHASE OF SERVICES CONTRACT BETWEEN WELD COUNTY
DEPARTMENT OF SOCIAL SERVICES AND WELD MENTAL HEALTH CENTER,
INC. , AND AUTHORIZE CHAIRMAN TO SIGN
WHEREAS, the Board of County Commissioners of Weld County,
Colorado, pursuant to Colorado statute and the Weld County Home
Rule Charter, is vested with the authority of administering the
affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Purchase of
Services Contract between the Weld County Department of Social
Services and Weld Mental Health Center, Inc. , and
WHEREAS , the term of said Contract is from March 1, 1990 ,
through February 28, 1991 , with the further terms and conditions
being as stated in the Contract, and
WHEREAS, after review, the Board deems it advisable to
approve said Contract, a copy being attached hereto and
incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County
Commissioners of Weld County, Colorado, that the Purchase of
Services Contract between the Weld County Department of Social
Services and Weld Mental Health Center, Inc. , be, and hereby is,
approved.
BE IT FURTHER RESOLVED by the Board that the Chairman be, and
hereby is, authorized to sign said Contract.
The above and foregoing Resolution was, on motion duly made
and seconded, adopted by the following vote on the 26th day of
February, A.D. , 1990 .

PURCHASE OF SERVICE CONTRACT
Mental Health Evaluations

2. This contract shall be effective from March 1 , 1990 until
February 28, 1991 .
3. County agrees to purchase and Contractor agrees to furnish (See Contract) of

per evaluation. The total amount of this contract may not exceed $ 8,000.00
4. County agrees:
a. To pay Contractor after satisfactory completion of one or more
evaluations and receipt of Contractor billing.
b. To determine the eligibility for services and provide information
regarding rights to fair hearings prior to referral to Contractor.
c. To assure that the service described herein has been accomplished and
a report received prior to payment to Contractor.
5. Contractor agrees:
a. Not to assign any provision of this contract to a subcontractor.
b. Not to charge clients any fees related to services provided under
this contract.
c. To hold the necessary license(s) which permits the performance of the
service to be purchased herein.
d. To comply with the requirements of the Civil Rights Act of 1964, and
Section 504, Rehabilitation Act of 1973 concerning discrimination on
the basis of race, color, sex, age, religion, political beliefs,
national origin, or handicap, and requirements for safeguarding client
information in accordance with rules of the State Department.
e. To provide access by county, state, and federal officials to such
records as are necessary to document the cost and delivery of
service.
f. To provide the service described herein at cost not greater than that
charged to other persons in the same community.
6. Termination: Either party may terminate this contract by 30 days prior
nojification in writing.

THIS AGREEMENT, made this 1st day of March, 1990 _ between the weld
County Department of Social Services, herein referred to as "County" and the
Weld Mental Health Center, Inc. , herein referred to as "Provider".
NOW THEREFORE, it is hereby agreed that in consideration of the mutual
undertakings and agreements hereinafter set forth, the County and the
Provider agree as follows:
1. Subject to its other provision, the term of this Agreement shall be
from the 1st day of March, 1990, through the 28th day of February, .
2. The County agrees to purchase and the Provider agrees to furnish
evaluations and related services listed below at the following rates:
ACTIVITY PERSONNEL RATE PER FOUR
Conference/Consultation Non-Psychologist $40.00
Psychologist $45.00
Individual/Family Assessment Non—Psychologist $40.00
Psychologist $45.00
Psychological Testing Psychologist $45.00
Psychiatric Testing Psychiatrist $55.00
Court Appearance Non—Psychologist $40.00
Psychologist $45.00
Psychiatrist $55.00
3. An Individual Evaluation will not exceed `$475.00 and a Family
Evaluation will not exceed $950.00 .
Individual and family evaluations that would exceed the above costs may
be negotiated and authorized prior to initiating the Evaluation.
4. The above rates would be used to determine total cost for each
referral. Rates are for total hours including contact with clients,
interpretation of results and development of reports.
5. Reimbursement will be for actual, reasonable and necessary costs
incurred for services described herein and will be made monthly upon
receipt of individual client costs on forms provided by the County.
IT IS FURTHER AGREED that either party shall have the right to
terminate this agreement by giving the other party thirty (30) days notice
in written form. If notice is given, this Agreement shall terminate on the
expiration of the thirty (30) days and the liability of the parties
hereunder for further performance of the terms of the Agreement shall
thereupon cease, but the parties shall not be released from the duty to
perform their obligation up to the date of termination.
UPON AGREEMENT, the parties herein signed this agreement to be
effective on the day and date written above.

The attached is a renewal contract between the two agencies listed above. ,
The annual budget for these services are $8,000. There are rate increases
which are acceptable to the Department and will be funded out of the
budgeted $8,000. The rate increases are for the following service areas:
