RE: APPROVE ADDITIONS TO FEE SCHEDULE FOR WELD COUNTY HEALTH
DEPARTMENT
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a request to add certain fees to the fee
schedule for the Weld County Health Department, and
WHEREAS, after review, the Board deems it advisable to approve said additional fees as
described in Exhibit A, a copy being attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the additions to the fee schedule for the Weld County Health Department,
as described in the above referenced Exhibit A, be, and hereby are, approved.
The above and foregoing Resolution was, on motion duly made and seconded, adopted by
the following vote on the 9th day of November, A.D., 1994.

•Subject: Addition of Drugs to Nursing Services
Enclosed for Board approval is a listing of new drugs the Nursing Division would like to provide
to clients. The fees for these drugs would be charged on a sliding fee scale as indicated in the
listing. Our practitioners feel these drugs have benefits over those currently being used and
would like to begin using them as soon as possible.
I recommend the Board's approval of these drugs and the sliding fee scale as shown..
