HomeMy WebLinkAbout920835.tiff RESOLUTION RE: APPROVE GRANT APPLICATION TO COMPRECARE FOUNDATION'S ELDERLY HEALTH PROMOTION INITIATIVE FROM WELD COUNTY HEALTH DEPARTMENT WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to Colorado statute and the Weld County Home Rule Charter, is vested with the authority of administering the affairs of Weld County, Colorado, and WHEREAS, the Board has been presented with a Grant Application to the Comprecare Foundation's Elderly Health Promotion Initiative from the Weld County Health Department's Suicide Prevention Coalition, commencing January 1, 1993, and ending December 31, 1993, with the further terms and conditions being as stated in said application, and WHEREAS, after review, the Board deems it advisable to approve said application, a copy of which is attached hereto and incorporated herein by reference. NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld County, Colorado, that the Grant Application to the Comprecare Foundation's Elderly Health Promotion Initiative from the Weld County Health Department's Suicide Prevention Coalition be, and hereby is, approved. BE IT FURTHER RESOLVED by the Board that the Chairman be, and hereby is, authorized to sign said application. Webster 920835 c c - Ht P C SUICIDE PREVENTION AMONG THE ELDERLY APPLICANT: Weld County Health Department CONTACT: Joann Harris, Project Coordinator, 356-0639 Suicide continues to be one of the top ten causes of death nationally as well as in Colorado. National rates for suicide in 1989 were 12.2/100,000 and • Colorado was ranked 8th highest in the nation at 16.6/100,000. The Weld County rate averaged over the three years (89-91) was 67.2 for those aged 65 and older. Colorado's population continues to age as does that of the rest of the nation. It is important that we develop successful programs to prevent suicide in this ever increasing population. 1993 GOALS AND OBJECTIVES (Changes for 1993 are underlined) I. Increase community awareness of suicide prevention issues A. Provide at least 15 educational presentations at sites additional to 1991 and 1992 trainings 1. Ten senior nutrition sites 2. Four senior caregiver groups 3. One general public presentation 4. Increase contact with Hispanic population II. Increase community and Coalition knowledge about completed suicides in Weld County A. Maintain working relationship with county coroner, first responder agencies and the media 1. Board meetings every other month 2. Increase press coverage of activities B. Network at national, state, and local levels to keep current in the field of suicide prevention 1. Maintain membership in the American Association of Suicidology 2. Increase state contacts and attend meetings C. Train and support a new Heartbeat facilitator 1. Purchase resource library materials for training and coordinating the survivors group 2. Seek financing to send a Heartbeat representative to AAS survivor' s conference 3. Provide a list of possible acquisitions to the county library III. Identify funding sources for continued support of the Coalition A. Continue to seek funding for Coalition activities from local community B. Increase community awareness on the suicide prevention issues * Developed directory of mental health providers willing to see suicidal clients and/or survivors * Provided 10 educational presentations to seniors and/or their community caregivers, 1 in Spanish * Sponsored a successful regional conference on suicide prevention (elderly emphasis) * Presented our project at the 1992 American Association of Suicidology Conference * Updated membership list * Reprinted and are distributing another 500 brochures "The Golden Years" * Updated fact sheet * Sent our Coalition newsletter II. Increase community and Coalition knowledge about completed suicides in Weld County * Presented 1991 and 1992 statistics by county coroner * Elected ambulance service director as Board Chair * Networking at national, state and local levels on statistics and prevention * Held annual Board retreat * Provided technical support to an area long term care facility and community college III. Identify funding sources for continuation of the project * Finalized 501(c) 3 paperwork * Received $1576 from local businesses and organizations for short term adjectives * Received in-kind donations of supervisors salary, additional staff benefits, office and overhead from Weld County Health Department worth a total of $ 6,176 * Received at least 300 hours of volunteer time for trainings, conferences, and Board business * Received donation of all speaker' s times for regional conference including key-note speaker * Received donated space for all Board and Heartbeat meetings. 1992 EXPENDITURES As of the end of June we were slightly over budget on salaries. This is off-set by July and August vacation time. Grant Application J Subject: Enclosed for Board approval is a grant application to the Comprecare foundation from the Weld County Health Department. The Health Department is applying for $10,000 in continuation funds to continue the initiatives of the Weld County Suicide Coalition. The Coalition will continue to increase our knowledge of completed suicides in Weld County and increase community awareness of suicide prevention issues. The Coalition has funded these efforts for the past two years. The grant period shall be from January 1, 1993 through December 31, 1993.
