RE: APPROVE CONTRACT FOR ACCESS TO BIRTH CERTIFICATE ISSUANCE SYSTEM AND AUTHORIZE
CHAIRMAN TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant
to Colorado statute and the Weld County Home Rule Charter, is vested with the
authority of administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with the Contract for Access to Birth
Certificate Issuance System with the Colorado Department of Health, with the further
terms and conditions being as stated in said Contract, and
WHEREAS, after review, the Board deems it advisable to approve said Contract,
a copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Contract for Access to Birth Certificate Issuance System
be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chairman be, and hereby is,
authorized to sign said Contract.
The above and foregoing Resolution was, on motion duly made and seconded,
adopted by

ACCESS TO THE BIRTH CERTIFICATION SYSTENI
Whereas the Colorado Department of Health operates a Birth Certificate Issuance data system that
approved local agencies are being granted access to for the purpose of issuing certificates to
their customers, and whereas our agency would like to have access to this system, we arc now
applying for such access and agree to the following:
We will pay an initial hook-up fee of $450 to cover the costs of necessary software for our
PC and for minor expansion hardware at the Colorado Department of Health.
If our access involves long-distance toll charges, we agree to reimburse the Department of
Health for its expenses related to calling our PC back. We agree to such reimbursement
until alternative access setups (such as fixed rate services at our agency) are agreed upon
and established.
We understand that the Department of Health is using principally existing communications
capacity to support the Birth Certificate access. If demand grows to the point where
existing capacity cannot adequately support needs, we agree to participate in discussions
regarding funding for capacity expansion

Contract with Colorado Department of Health for Access
Subject to Birth Certificate Issuance System
Enclosed for Board approval is the agreement between the Weld County Health
Department and the Colorado Department of Health.
This agreement formalizes the application made by Weld County to access and
tie into the Colorado Department of Health's Birth Certificate Issuance
system. This access was part of the approved computer budget for the year
1991.
