RE: APPROVE NOTIFICATION OF GRANT AWARD FOR ADMINISTRATION ON AGING MODEL
PROJECT GRANT AND AUTHORIZE CHAIRMAN TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado,
pursuant to Colorado statute and the Weld County Home Rule Charter, is vested
with the authority of administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Notification of Grant Award
for Administration on Aging Model Project Grant, commencing October 1, 1992, and
ending July 31, 1993, with the further terms and conditions being as stated in
said notification, and
WHEREAS, after review, the Board deems it advisable to approve said
notification, a copy of which is attached hereto and incorporated herein by
reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Notification of Grant Award for Administration on
Aging Model Project Grant be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chairman be, and hereby is,
authorized to sign said notification.
The above and foregoing Resolution was, on motion duly made and seconded,
adopted by the following vote on the 16th day of November, A.D. , 1992, nunc pro
tunc October 1, 1992.

The project application is an integral part of the NOGA and any changes
in the project application must be approved by Aging and Adult Services
prior to implementation. The award made available by this NOGA is
subject to the attached NOGA conditions.

GENERAL NOGA CONDITIONS
A. If costs claimed under this grant are disallowed as a result of an audit
or other financial or programmatic review,the proportion of Federal or
Federal/State funds disallowed shall be funded with 100%local funds.
B. The award made for this AOA model project are subject to and
contingent upon the availability of Federal and carry-over funds for the
purpose of the awards.
C. To the extent authorized by law,the grantee shall indemnify, save, and
hold harmless the State, its employees and agents, against any and all
claims, liability and court awards including costs, expenses and
attorney fees incurred as a result of any act or omission by the
grantee, or its employees, agents, subcontractors, subgrantees, or
assignees pursuant to the terms of this grant.

Enclosed for Board signature is a Notification of Grant Award in the amount of
$5,000.00 for FY 1992-1993 from the Administration on Aging. The purpose of this
award is to expand the national Project Care Initiative to Colorado and Weld
County.
The funds made available under this NOGA are to be used to develop leadership in
local communities to assist them to respond to the problems of their vulnerable
older citizens. The funds may not be used to purchase equipment or to pay for
salaries of Area Agency on Aging staff.
In Weld County the Area Agency on Aging will be working with community coalitions
in the Tri Town, Keenesburg, Hudson, and North Greeley communities to strengthen
their outreach programs to isolated and frail elders.
This grant will end July 31, 1993.
