RE: APPROVE COLORADO HEALTHY COMMUNITY INITIATIVE (CHCI) GRANT REQUEST BETWEEN
WELD COUNTY COMMUNITY HEALTH COALITION AND THE COLORADO TRUST AND
AUTHORIZE CHAIRMAN TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado,
pursuant to Colorado statute and the Weld County Home Rule Charter, is vested
with the authority of administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Colorado Healthy Communities
Initiative Grant Request between Weld County Community Health Coalition and The
Colorado Trust, with the terms and conditions being as stated in said grant
request, and
WHEREAS, after review, the Board deems it advisable to approve said grant
request, a copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Colorado Healthy Communities Initiative (CHCI) Grant
Request between Weld County Community Health Coalition and The Colorado Trust be,
and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chairman be, and hereby is,
authorized to sign said grant request.
The above and foregoing Resolution was, on motion duly made and seconded,
adopted by the following vote on the 14th day of September, A.D. , 1992.

5. OUTLINE OF OUTREACH STRATEGIES
The outreach and inclusion of as many facets as possible of our community
is one of the major activities of this Initiative. We expect to hire a part-
time staff person with the funds granted to us by The Trust. The staff will
contact local community members who may be interested or may know of others
interested in participating in the process. An effort will be made to attract
a diversity of representatives from communities and population groups
throughout Weld County. Through the cooperative extension we will reach some
of the more rural communities and organizations such as homemakers, 4-H clubs
and FHA. Greeley Interfaith and Ministerial Alliance will connect us with
community leaders in churches. Business representatives will be approached
through the Weld County Chamber of Commerce. Weld County Health Department's
well adult clinics and rural health clinics will be a contact for key
informants in outlying areas of the county. We will also seek representation
from at risk neighborhoods such as Island Grove in north Greeley. Youth will
be contacted through the school districts. An effort will be made to include
those in most need of health and human service support such as teen parents.
The media in Weld County has been very cooperative in past efforts at
community coalition building. The Greeley Tribune published results of United
Way's needs assessment in 1989. We expect to use The Greeley Tribune, the
cable news station, other local newspapers and local radio stations in
informing our community of the progress of Colorado Healthy Communities
Initiative.
Since Weld County is such a large area, monthly stakeholders meetings
will be rotated throughout the county. This will reduce some of the travel
burden for those living outside of the Greeley area and will also help signify
that this is a county initiative and not just a Greeley initiative. Part of
the staff person's responsibility will be to notify stakeholders and keep them
informed of the progress of the initiative through mailings and written
communication such as a CHCI newsletter.

Enclosed for Board approval is a grant request to The Colorado Trust from the
Weld County Community Health Coalition through the Weld County Health
Department.
The Coalition is requesting $7,500 in grant funds to initiate a health
planning effort in Weld County in order to develop community health priorities
and design projects to address those priority issues.
