RE: APPROVE THREE NOTIFICATION OF FINANCIAL ASSISTANCE AWARDS FOR PLACEMENT
ALTERNATIVES COMMISSION FUNDS FROM THE COLORADO DEPARTMENT OF SOCIAL
SERVICES AND AUTHORIZE CHAIRMAN TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado,
pursuant to Colorado statute and the Weld County Home Rule Charter, is vested
with the authority of administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with three Notification of Financial
Assistance Awards for Placement Alternatives Commission Funds from the Colorado
Department of Social Services for Weld Mental Health Center, Child Advocacy
Resource and Education, and Weld County Partners, with the terms and conditions
being as stated in said awards, and
WHEREAS, after review, the Board deems it advisable to approve said awards,
copies of which are attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, ex-officio Board of Social Services, that the three
Notification of Financial Assistance Awards for Placement Alternatives Commission
Funds from the Colorado Department of Social Services be, and hereby are,
approved.
BE IT FURTHER RESOLVED by the Board that the Chairman be, and hereby is,
authorized to sign said awards.
The above and foregoing Resolution was, on motion duly made and seconded,
adopted by the following vote on the 1st day of June, A.D. , 1992.

Alternatives Commission ( C)
DATE: May 29, 1992
Enclosed for Board approval are three Notification of Financial Assistance Awards
(NFAA) for Placement Alternatives Commission (PAC) funds. These awards are based
on a contract period of 6/1/92 - 5/31/93 and are subject to final adjustments in
funding by the Colorado Department of Social Services estimated to be determined
in July - August 1992. This approval action of the NFAAs will conclude the
Request for Proposal Process undertaken by the Weld County Department of Social
Services and the Placement Alternative Commission.
The description of the NFAAs is as follows:
1. Weld Mental Health Center will be initially awarded $31,893.40 at an
hourly rate of $10.22. The Mental Health Center program will serve up to
four families per month for a period of 12/1/92 - 5/31/93.
2. Child Advocacy Resource and Education (CARE) will be initially awarded
$28,800.00 per year at an hourly rate of $3.96. The CARE program will
serve up to 18 families monthly for one year.
3. Weld County Partners will be initially awarded $105,170.00 per year at an
hourly rate of $10. 11.
The Partner's program will serve up to 25 children per month.
