RE: APPROVE FAMILY PLANNING CONTRACT WITH THE COLORADO DEPARTMENT OF HEALTH
AND AUTHORIZE CHAIRMAN TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado,
pursuant to Colorado statute and the Weld County Home Rule Charter, is vested
with the authority of administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Family Planning Contract
between the Colorado Department of Health and the Weld County Health Department,
commencing July 1, 1992, and ending June 30, 1993, with the further terms and
conditions being as stated in said contract, and
WHEREAS, after review, the Board deems it advisable to approve said
contract, a copy of which is attached hereto and incorporated herein by
reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Family Planning Contract between the Colorado
Department of Health and the Weld County Health Department be, and hereby is,
approved.
BE IT FURTHER RESOLVED by the Board that the Chairman be, and hereby is,
authorized to sign said contract.
The above and foregoing Resolution was, on motion duly made and seconded,
adopted by the following vote on the 27th day of July, A.D. , 1992, nunc pro tunc
July 1, 1992.

Dear Dr. Randy Gordon:
The Colorado Department of Health hereby notifies Weld County Health
Department that the contract amount from the Family Planning Program, Contract
# FPP 920014 will be One hundred forty-four thousand five hundred fifty-three
dollars ($144,553) for the period July 1, 1992 through June 30, 1993. One
hundred and six thousand eight hundred twenty-eight dollars ($106.828) shall
be used to provide contraceptive services to 1650 comprehensive family
planning patients; Thirty-two thousand eight hundred fifty dollars (S32.850)
shall be used to provide colposcopy and biopsy services to 150 patients; and
Four thousand eight hundred seventy-five dollars ($4.875) shall be used to
provide cryosurgery services to 65 patients.
This notification shall not be deemed valid until it is approved by the
controller of the State of Colorado or his designee. Please sign and return
all copies of this notification to the Family Planning Program. A fully
executed copy will be returned to you.

Subject: Family Planning Contract
Enclosed for Board approval is a contract between the Weld County Health
Department (WCHD) and the Colorado Department of Health for our Family Planning
Program.
The WCHD will be reimbursed $106,828 to provide contraceptive services to
approximately 1,650 comprehensive family planning patients, $32,850 to provide
colposcopy and biopsy services to 150 patients, and $4,875 to provide cryosurgery
services to 65 patients for a total reimbursement of $144,553.00. This amount
is an increase of $13,001 for family planning services from the prior year's
contract.
The WCHD is responsible for conducting a comprehensive family planning program
in compliance with Title X and Colorado Department of Health regulations.
The term of the contract shall be from July 1, 1992 through June 30, 1993.
