RE: APPROVE REVISION TO NOTIFICATION OF FINANCIAL ASSISTANCE AWARD FOR
PLACEMENT ALTERNATIVES COMMISSION FUNDS WITH NORTH COLORADO PSYCHCARE AND
AUTHORIZE CHAIRMAN TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado,
pursuant to Colorado statute and the Weld County Home Rule Charter, is vested
with the authority of administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Revision to the Notification
of Financial Assistance Award for Placement Alternatives Commission Funds between
North Colorado PsychCare and the Weld County Department of Social Services,
commencing January 20, 1994, and ending May 31, 1994, with further terms and
conditions being as stated in said notification revision, and
WHEREAS, after review, the Board deems it advisable to approve said
notification revision, a copy of which is attached hereto and incorporated herein
by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, ex-officio Board of Social Services, that the Revision to the
Notification of Financial Assistance Award for Placement Alternatives Commission
Funds between North Colorado PsychCare and the Weld County Department of Social
Services be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chairman be, and hereby is,
authorized to sign said notification revision.
The above and foregoing Resolution was, on motion duly made and seconded,
adopted by the following vote on the 7th day of February, A.D. , 1994, nunc pro
tunc January 20, 1994.

SUBJECT: Revision to Notification of Financial Assistance Award (NOFA)
between Youth Passages and the Weld County Department of Social
Services
Enclosed for Board approval is a Revision to the NOFA between Youth Passages
and the Weld County Department of Social Services for Placement Alternatives
Commission (PAC) funds.
The purpose of this revision is to increase the yearly budget of Youth
Passages from $37,740.88 to $42,740.88. The difference being $5,000.
This revision does not change any of the other original obligations or
provisions.
