Subwct: Purchase of Services Agreement Amendment between Human Resources'
Senior Nutrition Program and University of Northern Colorado
Enclosed for Board approval is a Purchase of Services Agreement between
the Weld County Senior Nutrition Program and the University of Northern
Colorado Food Service.
This agreement allows the Weld County Senior Nutrition Program to purchase
approximately 70,000 meals annually for $2.81 per meal to be provided to
over 2,000 seniors annually throughout Weld County.

Purchase of Services Agreement Amendment
THIS AGREEMENT, made and entered into this 21st day of March , 1990
by and between the Weld County Division of Human Resources ' Senior Nutrition
Program, hereinafter referred to as the "Program," and the University of
Northern Colorado, hereinafter designated as the "Vendor."
WITNESSETH:
WHEREAS, the "Program" desires to continue its contractual arrangement for
meals from the "Vendor", and
WHEREAS, the "Program" and the "Vendor" are desirous of amending the
original agreement dated February 16, 1987, and the amendment dated December
30, 1987.
NOW THEREFORE, it is mutually agreed:
1. The "Program" will reimburse the "Vendor" $2.81 for a complete meal
including I pint of milk, based on an annual evaluation of the Consumer Price
Index.
2. The cost of raw food including 1/2 pint of milk will not exceed $1.10
per serving.
3. All other terms and conditions of the original agreement shall remain
in effect unless otherwise stated in this Amendment.
4. Item 15 of the original contract is amended to also read that the
Vendor will notify the program one year prior to the December, 1992 contract
expiration date in the event that the Vendor chooses not to submit a bid to
provide meals for the Senior Nutrition Program.
5. Item 8 and 9 of original contract is amended to also read:
(8) Weld County Division of Human Resources agrees to provide labor
assistance through Employment and Training programs upon availability of
employees and funding.

(9) The vendor agrees to deduct from the monthly billing the
equivalent of hourly minimum wage for the four (4) work experience positions
provided by the Division of Human Resources.
IN WITNESS WHEREOF, the parties hereto have executed this Amendment on the
day first written above.
