RE: APPROVE PURCHASE OF SERVICES AGREEMENT AMENDMENT BETWEEN SENIOR NUTRITION
AND UNIVERSITY OF NORTHERN COLORADO AND AUTHORIZE CHAIRMAN TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado,
pursuant to Colorado statute and the Weld County Home Rule Charter, is vested
with the authority of administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Purchase of Services Agreement
Amendment between the Weld County Division of Human Resources' Senior Nutrition
Program and the University of Northern Colorado, with the terms and conditions
being as stated in said agreement amendment, and
WHEREAS, after review, the Board deems it advisable to approve said
agreement amendment, a copy of which is attached hereto and incorporated herein
by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Purchase of Services Agreement Amendment between the
Weld County Division of Human Resources' Senior Nutrition Program and the
University of Northern Colorado, be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chairman be, and hereby is,
authorized to sign said agreement amendment.
The above and foregoing Resolution was, on motion duly made and seconded,

PURCHASE OF SERVICES AGREEMENT AMENDMENT
THIS AGREEMENT, made and entered into this 1st day of February, 1992, by
and between the County of Weld, Sate of colorado, by and through the Board of
County Commissioners of Weld County, on behalf of the Weld County Division of
Human Resources' Senior Nutrition Program, hereinafter referred to as "Program,"
and the University of Northern Colorado, hereinafter referred to as the "Vendor."
WITNESSETH:
WHEREAS, the "Program" desires to continue its contractual arrangement for
meals from the "Vendor", and
WHEREAS, the Program" and the "Vendor" area desirous of amending the
original agreement dated February 16, 1987, and the amendment dated February,
1991,
NOW THEREFORE, it is mutually agreed:
1. The "Program" will reimburse the "Vendor" $3.00 for a complete meal
including 1/2 pint of milk, based on the annual evaluation of the Consumer Price
Index.
2. The cost of the raw food including 1/2 pint of milk will not exceed
$1.20 per serving.
3. The "Vendor" will notify the "Program" one year prior to the December,
1992 contract expiration date in the event that the Vendor chooses not to submit
a bid to provide meals for the Senior Nutrition Program.
4. All other terms and conditions of the original agreement shall remain
in effect unless otherwise stated in this Amendment.
IN WITNESS WHEREOF, the parties hereto have executed this Amendment on the
day first written above.

Enclosed for Board approval are the following Purchase of Services Agreements:
1. University of Northern Colorado - This is an Amendment to the original
Purchase of Services Agreement changing the cost per meal for the Senior
Nutrition Program to $3.00 per meal including milk. This amendment is in
effect until January 31, 1993.
2. Larimer County Ombudsman Program - This agreement allows the Larimer
County Nursing Home Ombudsman to respond to complaints filed against the
Weld County Home and Community Based Services' Case Management Program.
The Weld County Ombudsman supervises the HCBS Program and is unable to
respond to HCBS complaints due to a conflict of interest.
Human Resources will reimburse Larimer County at $12.50 per hour including
travel time. Mileage will be reimbursed at twenty cents ( .20) per mile.
This Agreement will end December 31, 1992.
