RE: APPROVE AMENDMENT TO AGREEMENT FOR SHERIFF'S OFFICE MEDICAL UNIT SERVICE
AND AUTHORIZE CHAIRMAN TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado,
pursuant to Colorado statute and the Weld County Home Rule Charter, is vested
with the authority of administering the affairs of Weld County, Colorado, and
WHEREAS, on September 11, 1988, the Board did enter into a Medical Unit
Service Agreement with Preferred Correctional Medical Team, Inc. , and
WHEREAS, the Board has been presented with an Amendment to Agreement for
Sheriff's Office Medical Unit Service, commencing January 1, 1993, and ending
December 31, 1993, with the further terms and conditions being as stated in said
amendment, and
WHEREAS, after review, the Board deems it advisable to approve said
amendment, a copy of which is attached hereto and incorporated herein by
reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Amendment to Agreement for Sheriff's Office Medical
Unit Service between Weld County and Preferred Correctional Medical Team, Inc.
be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chairman be, and hereby is,
authorized to sign said amendment.
The above and foregoing Resolution was, on motion duly made and seconded,
adopted by the following vote on the 23rd day of November, A.D. , 1992.

AMENDMENT TO
AGREEMENT FOR WELD COUNTY SHERIFF OFFICE'S
MEDICAL UNIT SERVICE
THIS AMENDMENT, dated for reference purposes only the 31st day
of December, 1992, is made and entered into by and between the board
of County Commissioners of Weld County ( "County" ) and Preferred
Correctional Medical Team, Inc. ( "Provider" ) . It is hereby attatched
to and shall become a part of the Agreement by and between the County
and Provider which was executed September 11, 1988.
WITNESSETH:
WHEREAS, the County and Provider entered into a Medical Unit
Service Agreement ( "Agreement" ) September 11, 1988; and
WHEREAS, pursuant to Paragraphs VI and XVIII of the Agreement,
the parties may renew and renegotiate the payment provision of the
Agreement prior to the commencement of the new term for the following
year; and
WHEREAS, the Sheriff of Weld County desires the Agreement be
renewed and has negotiated the following changes, subject to approval
of the Board of County Commissioners.
NOW, THEREFORE, for good and valuable consideration, the receipt
and sufficiency of which are hereby acknowledged, County and Provider
agree to the following:
1. The term of the Agreement is hereby renewed and shall
commence January 1, 1993, and continue to and including
December 31, 1993.
2. The total combined cost limitation ceiling for outside
medical services including emergency room, clinic, dentist,
physician, consultants, and hospital, as described in
paragraph 3 of Amendment dated September 11, 1989, shall be
reduced from $36, 000 to $24, 000 per year commencing January
1, 1993. (The costs of the jail dentist and jail physician
shall not be included in arriving at the limitation amount. )
3. The monthly payment amount of $28, 000. 00 is hereby adjusted
to $30, 000. 00 for the period January 1 through December 31,
1993.
4. The Agreement, as amended hereby, shall remain in full force
and effect through December 31, 1993.
IN WITNESS WHEjEOF, the parties hereto have executed this
Amendment this day of November, 1992.

Dear Rick:
Enclosed are two copies of an Amendment to our Agreement with
Weld County for medical services at the Weld County Jail.
After approval by the commissioners, please return one copy
to my office.
I want you to know we really appreciate the cooperative
working relationship we have enjoyed with the Weld County
staff during the past 4 years. Perhaps more importantly to
Weld County, we hope you know that we are trying to very hard
to provide your jail with the quality of medical services
required to adeguately administer to the inmates' needs and
to do so at a reasonable price. We have maintained the same
` jail' physician and dentist, the same nursing director and
some of the original staff, and accreditation since we
entered into our original agreement in 1988. We believe that
continuity and our low turnover generally results in quality
service at a reasonable cost.
Medical service costs are increasing at an average rate of 12
to 14 percent annually. In a letter to David Shelton dated
June 23, 1992, we projected that our increase to Weld County
for 1993 would approximate eleven ( 11 ) percent. We are happy
that we are able to keep our increase to seven (7) percent.
To do so, we are proposing that the $36, 000 ceiling for
outside medical costs be decrease to $24. 000. Historically,
your annual outside medical costs have exceeded $24, 000 about
half the time.

Dear Lt. Shelton:
By this letter we are giving you the projected e 'cimate
of our rate increase for medical services at the Jail
for 1993. We understand that you need this estimate at
now so the Department can commence its 1993 budget
process for the County.
At this time, we are projecting an increase of eleven
( 11%) percent in our fee for 1993.
As you are aware, our outside medical costs for Weld
County Jail during the first 5 months of 1992 exceeded
the costs for the entire year in previous periods.
