RE: APPROVE AMENDMENT TO AGREEMENT FOR INTAKE SCREENING OF WELD COUNTY
JUVENILES IN CUSTODY AND AUTHORIZE CHAIRMAN TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado,
pursuant to Colorado statute and the Weld County Home Rule Charter, is vested
with the authority of administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with an Amendment to Agreement for
Intake Screening of Weld County Juveniles in Custody with Weld Adolescent
Resources, Inc. , with terms and conditions being as stated in said amendment, and
WHEREAS, after review, the Board deems it advisable to approve said
amendment, a copy of which is attached hereto and incorporated herein by
reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Amendment to Agreement for Intake Screening of Weld
County Juveniles in Custody with Weld Adolescent Resources, Inc. be, and hereby
is, approved.
BE IT FURTHER RESOLVED by the Board that the Chairman be, and hereby is,
authorized to sign said amendment.
The above and foregoing Resolution was, on motion duly made and seconded,
adopted by the following vote on the 20th day of October, A.D. , 1993.

AMENDMENT TO AGREEMENT FOR INTAKE SCREENING
OF WELD JUVENILES IN CUSTODY
THIS AMENDMENT TO AGREEMENT, made and entered into this 9
day of October, 1993, by and between Weld County Colorado, a body
corporate and politic of the state of Colorado, by and through the
Board of County Commissioners of Weld County, on behalf of the Weld
County Sheriff, whose address is 915 Tenth Street, Greeley,
Colorado 80631, hereinafter referred to as "County, " and Weld
Adolescent Resources, Inc. , whose address is 3412 Lupton Avenue,
Evans, Colorado 80620, hereinafter referred to as "Contractor. "
WITNESSETH:
WHEREAS, on June 30, 1993 County and Contractor entered into
an Agreement for Intake Screening of Weld County Juveniles in
Custody, a copy of which is attached hereto, and
WHEREAS, there is now a need as expressed by both parties
hereto to delete the electronic monitoring portion of said
Agreement and to alter the monthly compensation for the services as
supplied by Contractor, and
WHEREAS, the following Amendment to said Agreement for Intake
Screening of Weld County Juveniles in Custody is in the best
interests of the people of the County of Weld, State of Colorado.
NOW, THEREFORE, in consideration of the mutual promises and
covenants contained herein, the parties hereto agree to amend the
Agreement for Intake Screening of Weld County Juveniles in Custody
as executed on June 30, 1993, by and between the parties hereto, as
follows :
A. Delete all of Section 1 entitled "SERVICES TO BE
RENDERED" as found on page 1 of said Agreement and insert
the following:
1 . SERVICES TO BE RENDERED:
Contractor agrees to perform professional
services set forth in Exhibit "A"
entitled "Proposal Pertaining to Intake
Screening of Weld County Juveniles in
Custody" and dated May 26, 1993, which is
attached hereto and is made a part of
this Agreement, with the exception of any
references to "electronic monitoring" ,
"electronic home monitoring, " and/or
"home detention. " All work performed by
and under the direction of the Contractor
shall be consistent with the highest
standards of care and in strict compli-
ance of all applicable federal, state,
and local laws, regulations, and proce-
dures which may from time to time be

adopted or amended. "Reasonable amount
of time" as set forth on page 2 of
Exhibit "A" under "Temporary Staff Super-
vision" shall be defined as a period of
time not to exceed 1 . 25 hours per
juvenile.
B. Delete all of Section 2 entitled "TERM OF AGREEMENT" and
found on page 1 of said Agreement, and insert the
following:
2 . TERM OF AGREEMENT:
This Agreement shall be effective
November 1, 1993 and shall terminate on
June 30, 1994, unless otherwise termi-
nated as set forth herein.
C. Delete all of Section 4 entitled "PROFESSIONAL FEES" as
found on page 2 of said Agreement and insert the
following:
4 . PROFESSIONAL FEES:
In consideration of the terms and obliga-
tions of this Agreement, County agrees to
pay and Contractor agrees to accept, as
full and complete compensation for all
services rendered under this Agreement, a
total sum not to exceed $32, 942 . 00,
payable as follows :
a. Equal monthly installments of
$4 , 117 . 75, due and payable on the
last day of each month, beginning
November 30, 1993 and continuing to
and until June 30, 1994 .
b. The consideration set forth in
subparagraph "a" above shall be the
entire amount of compensation to be
paid by County for the services
contemplated herein. Contractor
agrees that any modification or
revision within the annual program
budget in excess of 10 percent by
existing categories shall be
submitted to County for written
approval prior to implementation by
Contractor.
c. From July 6, 1993 to October 31,
1993 County has paid to Contractor
the sum of $34, 065 . 36 for all
services rendered during those four
months pursuant to the Agreement

which is attached. The parties
agree that the sum of $34,065 . 36 is
the entire amount of compensation
which Contractor was due for all
services rendered pursuant to said
Agreement to and until October 31,
1993 .
D. Delete all references to "electronic monitoring, "
"electronic home monitoring, " and/or "home detention" as
stated in Exhibit "A" to the Agreement which is attached
hereto.
IN WITNESS WHEREOF, the pares hereto have executed this
