RE: APPROVE REVISION TO NOTIFICATION OF FINANCIAL ASSISTANCE AWARD FOR
PLACEMENT ALTERNATIVES COMMISSION FUNDS FOR NORTH COLORADO PSYCHCARE AND
AUTHORIZE CHAIRMAN TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado,
pursuant to Colorado statute and the Weld County Home Rule Charter, is vested
with the authority of administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Revision to the Notification
of Financial Assistance Award for Placement Alternatives Commission Funds between
the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Department of Social Services, and
North Colorado Psychcare, with terms and conditions being as stated in said
notification, and
WHEREAS, after review, the Board deems it advisable to approve said
notification, a copy of which is attached hereto and incorporated herein by
reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, ex-officio Board of Social Services, that the Revision to the
Notification of Financial Assistance Award for Placement Alternatives Commission
Funds between the County of Weld, State of Colorado, by and through the Board of
County Commissioners of Weld County, on behalf of the Department of Social
Services, and North Colorado Psychcare be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chairman be, and hereby is,
authorized to sign said notification.
The above and foregoing Resolution was, on motion duly made and seconded,
adopted by the following vote on the 23rd day of May, A.D. , 1994.

SUBJECT: Revision to Notification of Financial Assistance Award (NOFA)
Between North Colorado Psychcare and Weld County Department of
Social Services
Enclosed for Board approval is a Notification of Financial Assistance Award
(NOFA) between the North Colorado Psychcare and Weld County Department of Social
Services for Placement Alternatives Commission (PAC) funds.
The purpose of this revision is to increase the yearly budget of North Colorado
Psychcare from $42,740.88 to $52,740.88. The difference being $10,000.00
This revision does not change any of the other original obligations or
provisions.
