RE: APPROVE FAMILY PLANNING PROGRAM CONTRACT AMENDMENT BETWEEN WELD COUNTY
HEALTH DEPARTMENT AND STATE DEPARTMENT OF HEALTH AND AUTHORIZE CHAIRMAN TO
SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado,
pursuant to Colorado statute and the Weld County Home Rule Charter, is vested
with the authority of administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Family Planning Program
Contract Amendment between Weld County Health Department and State Department of
Health, commencing July 1, 1991, and ending June 30, 1993, with the further terms
and conditions being as stated in said contract amendment, and
WHEREAS, after review, the Board deems it advisable to approve said
contract amendment, a copy of which is attached hereto and incorporated herein
by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Family Planning Program Contract Amendment between
Weld County Health Department and State Department of Health be, and hereby is,
approved.
BE IT FURTHER RESOLVED by the Board that the Chairman be, and hereby is,
authorized to sign said contract amendment.
The above and foregoing Resolution was, on motion duly made and seconded,
adopted by the following vote on the 18th day of March, A.D. , 1992, nunc pro tunc
July 1, 1991.

The Colorado Department of Health hereby notifies Weld County Health
Department that the maximum amount of reimbursement from the Family Planning
Program, Contract No. FPP920014, covering the period July 1, 1991 to June 30,
1993, is hereby increased by Twenty Nine Thousand Four Hundred Fifty Two
Dollars and 50 Cents ($29,452.50) for colposcopy and biopsy services; and
increased by Five Thousand Three Hundred Twenty Five Dollars ($5.325) for
cryosurgery services, to a new total of One Hundred Thirty Six Thousand Eight
Hundred Seventy Seven Dollars and 50 Cents ($136,877.50) (Page 3, Paragraph
4) . The total number of patients to be served for colposcopy and biopsy is to
be increased by One Hundred Sixty Five (165) to a new total of One Hundred
Ninety Eight (198) ; and for cryosurgery increase Seventy One (71) to a new
total of Ninety (90) patients (Page 3, Paragraph 4) .

will be returned to you.

Subject: Family Planning Contract Amendment
Enclosed for Board approval is a contract amendment for the Weld County Health
Department's Family Planning Program. The Health Department will receive an
additional $29,452.00 to serve an additional 33 clients with colposcopy biopsy
services and an additional $5,325.00 to serve an additional 19 clients with
cryosurgery services.
The amendment covers the time period of July 1, 1991 through June 30, 1993.
