RE: APPROVE AMENDMENT TO PRENATAL PROGRAM CONTRACT WITH COLORADO DEPARTMENT OF
HEALTH AND AUTHORIZE CHAIRMAN TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado,
pursuant to Colorado statute and the Weld County Home Rule Charter, is vested
with the authority of administering the affairs of Weld County, Colorado, and
WHEREAS, by Resolution dated November 9, 1992, nunc pro tunc October 1,
1992, the Board approved the Prenatal Program Contract between the Colorado
Department of Health and the Weld County Health Department, and
WHEREAS, the Board has been presented with an Amendment to the Prenatal
Program Contract between the Colorado Department of Health and the Weld County
Health Department, commencing October 1, 1992, and ending September 30, 1993,
with the further terms and conditions being as stated in said contract, and
WHEREAS, after review, the Board deems it advisable to approve said
contract, a copy of which is attached hereto and incorporated herein by
reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Amendment to the Prenatal Program Contract between the
Colorado Department of Health and the Weld County Health Department be, and
hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chairman be, and hereby is,
authorized to sign said contract.
The above and foregoing Resolution was, on motion duly made and seconded,
adopted by the following vote on the 19th day of May, A.D. , 1993, nunc pro tunc

The Colorado Department of Health hereby notifies Weld County Health
Department that the-maximum amount of reimbursement from the Prenatal Program,
Contract No. FAA PRN930564, covering the period October 1, 1992 to September
30, 1993, is hereby increased by Nineteen Thousand Two Hundred-fifty Dollars
(S19 ,250.00) to a new total of Sixty-four Thousand. fifty Dollars (664.050)
(Page 3, Paragraph 10) .
The number of patients to be served is to be increased by 30 to a total of 72
patients, home visits are to be increased by 30 to a new total of 230 and
diagnostic test dollars are to be increased by $5,000.00 to a new total of
$10,000.00.
This notification shall not be deemed valid until it is approved by the
controller of the State of Colorado or his designee. Please sign and return
all copies of this notification. A fully executed copy will be returned to
you.

Subject: Amendment to Prenatal Program Contract
Enclosed for Board approval is an amendment to the contract between the Weld
County Health Department and the Colorado Department of Health for our Prenatal
Program.
The Health Department will receive an additional $19,250 to bring the total
contract reimbursement to $64,050 for the period October 1, 1992 through
September 30, 1993. The Health Department will provide prenatal care for an
additional 42 non-Medicaid clients, 30 additional home visits and additional
diagnostic services in the amount of $5,000.
I would recommend approval of this contract amendment.
