RE: APPROVE EXTENSION OF PURCHASE OF SERVICES AGREEMENT BETWEEN COLORADO
INCINERATION SERVICES, INC. , AND WELD COUNTY HEALTH DEPARTMENT AND
AUTHORIZE CHAIRMAN TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado,
pursuant to Colorado statute and the Weld County Home Rule Charter, is vested
with the authority of administering the affairs of Weld County, Colorado, and
WHEREAS, on June 24, 1991, the Board approved a Purchase of Services
Agreement between Colorado Incineration Services, Inc. , and the Weld County
Health Department, and
WHEREAS, the Board has been presented with an Extension to said agreement,
commencing May 1, 1992, and ending April 30, 1995, with the further terms and
conditions being as stated in said extension, and
WHEREAS, after review, the Board deems it advisable to approve said
extension, a copy of which is attached hereto and incorporated herein by
reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Extension to the Purchase of Services Agreement
between Colorado Incineration Services, Inc. , and the Weld County Health
Department be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chairman be, and hereby is,
authorized to sign said extension.
The above and foregoing Resolution was, on motion duly made and seconded,
adopted by the following vote on the 11th day of May, A.D. , 1992.

Re: Purchase of Services Agreement, made and entered into the 24th
day of June, 1991, by and between Colorado Incineration Services, Inc. ,
hereinafter referred to as "CI Services" and the County of Weld, State
of Colorado, by and through the Board of Commissioners of Weld County,
on behalf of the Weld County Health Department, hereinafter referred to
as "Health Department".
Dear Mr. Persichino:
Pursuant to our phone conversation, CI Services is hereby offering
an extension of the above referenced Agreement under the same terms,
fees and conditions of such Agreement for three (3) years commencing May
1, 1992 and expiring April 30, 1995.
If the above meets with your approval, please execute in the space
provided below and return one original copy to the above address.
We here at Colorado Incineration appreciate the opportunity to
serve your biomedical waste disposal needs.
