RE: APPROVE REVISION TO NOTIFICATION OF FINANCIAL ASSISTANCE AWARD FOR WELD
COUNTY PARTNERS AND AUTHORIZE CHAIRMAN TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado,
pursuant to Colorado statute and the Weld County Home Rule Charter, is vested
with the authority of administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Revision to the Weld County
Department of Social Services Notification of Financial Assistance Award for Weld
County Partners, Placement Alternatives Commission Funds, with the terms and
conditions being as stated in said revision, and
WHEREAS, after review, the Board deems it advisable to approve said
revision, a copy of which is attached hereto and incorporated herein by
reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, ex-officio Board of Social Services, that the Revision to the
Weld County Department of Social Services Notification of Financial Assistance
Award for Weld County Partners, Placement Alternatives Commission Funds, be, and
hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chairman be, and hereby is,
authorized to sign said revision.
The above and foregoing Resolution was, on motion duly made and seconded,
adopted by the following vote on the 20th day of July, A.D. , 1992.

RE: Revision to the ti ' atio of F nancial Assistance Award (NFAA) For
Partners
Enclosed for Board approval is a minor revision to the Notification of
Financial Assistance Award (NFAA) prepared for the Partners.
The original NFAA was issued on 06/01/92 and obligated Placement Alternatives
Commission (PAC) funds of $105,170 for a year. This revision changes the
hourly rate from $10.11 to $12.52 and the unit of service definition as
follows: The match of one advocate or volunteer with child for 25 hours per
month for up to 12 months (8.75 hours per week) . This revision does not
change the monthly and yearly budget.
