RE: APPROVE CONTRACT FOR VISION CARE WITH WELD COUNTY ASSOCIATION
OF OPTOMETRISTS AND OPHTHALMOLOGISTS AND AUTHORIZE CHAIRMAN
TO SIGN
WHEREAS , the Board of County Commissioners of Weld County,
Colorado, pursuant to Colorado statute and the Weld County Home
Rule Charter, is vested with the authority of administering the
affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Contract for
Vision Care with the Weld County Association of Optometrists and
Ophthalmologists, and
WHEREAS, the terms and conditions are as stated in said
Contract, a copy of which is attached hereto and incorporated
herein by reference, and
WHEREAS, after study and review, the Board deems it advisable
to approve said Contract.
NOW, THEREFORE, BE IT RESOLVED by the Board of County
Commissioners of Weld County, Colorado, that the Contract for
Vision Care with the Weld County Association o£ Optometrists and
Ophthalmologists be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chairman be, and
hereby is, authorized to sign said Contract.
The above and foregoing Resolution was, on motion duly made
and seconded, adopted by the following vote on the 17th day of
December, A.D. , 1990 .

_CONTRACT FOR VISION CARE - tg91
THIS AGREEMENT made and entered into this J�-day of December, 1990, by and between the
r-LARD OF COUNTY COMMISSIONERS OF THE COUNTY OF WELD, STATE OF COLORADO, Hereinafter referred
to as "County" and WELD COUNTY ASSOCIATION OF OPTOMETRISTS AND OPHTHALMOLOGISTS, Hereinafter
referred to as "Association".
WITNESSETH:
For and in consideration of the mutual promises and obligations set forth herein$ the parties
hereto mutually agree as follows:
1. Purpose.
The purpose of this Agreement is to set forth the framework under which the Vision Care
Program of the Weld County Department of Social Services shall be administered. The sum of
$17,000.00 has been appropriated by County for said program for calendar year 1991. It is
the mutual agreement and understanding of the parties hereto that the total amount to be paid
by County during calendar year 1991 shall not exceed said $17,000.00 and that when the budgeted
amount is depleted, no provision shall be made for further vision care services.
Participating optometrists and ophthalmologists, as well as social services staff,must monitor
cooperatively the use of this program in order to utilize the limited funds effectively and
efficiently.
2. Eligible Recipients.
County shall be responsible for payment for eyeglasses and services provided by members of
the Association to eligible recipients only after County has provided prior written
authorization for such services to a member of the Association.
Eligibility for such services shall be within the sound discretion of the County, but shall be
available only where alternative resources such as MEDICAID are not available.
3. Qualifications of Providers.
Only members of Association who are in good standing in the respective accredited society
and/or Association shall receive appointments as participating members of County's Vision
Care Program.
4. Fees for Services.
The following schedule sets for forth the reasonable fees which members of Association may
charge County and/or recipients/patients during the term of this contract:
A. The recipient/patient shall pay a $6.00 fee for his examination plus, if glasses are
prescribed and ordered, a $6.00 fee for the glasses, and all other costs that exceed the
maximum allowances before receipt of the lenses and frames.
B. If the recipient chooses deluxe/photogray lenses, any additional costs for such lenses
shall be paid by recipient, and in no event shall such additional charges be paid by
county. If recipient's choice of lenses or frames exceeds the standard allowance by
more than $30.00 that recipient must assume responsibility for the entire amount due,
with no payment made by County.
C. The basic examination fee shall be $34.00, of which County shall pay $28.00 and the
recipient/patient shall pay $6.00.
D. If tonometry is essential and is performed, the examining doctor shall bill County an
additional $4.00 for this service.
E. The costs of lenses shall be billed to County according to the following schedule:
(1) Single Vision Lenses $12.00
(2) Bifocal Lenses $17.00
(3) Trifocal Lenses $22.00
A laboratory prescription fee, based on prices for lenses from the current DUFFENS
Price List shall be paid by County in addition to the schedule set forth above.
F. County shall pay the sum of $12.00 as a standard frame service allowance, and in
addition, shall pay the actual laboratory cost of the frame up to a maximum of $15.00
based on the manufacturer's current price list. If a lower priced frame is used, County
shall be billed the lower cost. If a higher priced frame is used, County shall be
billed the maximum allowance of $15.00 and the recipient shall pay costs exceeding that
amount.

G. Temples and minor fram, repairs not exceeding $7.50 cos, shall be billed to County
at their customary charge. Lens and frame replacements are to be billed as set
- forth above.
5. Adjudicating Committee.
A committee consisting of representatives of Association shall recommend policies and
initiate procedures for this Vision Care Program and shall review billings and adjudicate,
consistent with this Agreement, any unusual fees. The billings shall be reviewed and
adjudicated by the committee on a bi-monthly basis through the month of September, after
which the committee shall review billings on a monthly basis in October, November, and
December.
6. Submittal of Vouchers.
When submitting vouchers for payment, members of Association must include with such
vouchers the authorization form set forth in paragraph 2 above and shall itemize all costs.
Together with itemized fees for services and laboratory costs, such statement must show
recipient's name, prescription, type of lens provided (single-vision, bifocal, trifocal)
and the amounts paid by the recipient/patient. Failure to comply with these provisions
shall result in disapproval of the charges by the Adjudicating Committee.
7. Members of Association - Independent Contractors.
It is expressly understood by the parties hereto that members of Association are not the
employees of County, but are independent contractors and shall be solely responsible for
their actions and those of their employees.
8. Amendment.
This Agreement may be amended by the parties hereto at any time, provided such amendment
is reduced to writing, signed by both parties and incorporated herein as an amendment.

1991 Contracts for Dental Care, Vision Care, Weld County

Please find attached the 1991 continuation contracts for dental care and
vision care. These contracts are funded from our General Assistance
program. The amounts approved by the County Board for 1991 are $17,000 for
each contract.
Within the Vision Care contract there have been some fee increases to
provide this service; however, all costs for the calendar year will be
limited to $17,000 available in the budget. The Dental Care contract is
literally the same as it was in the 1990 calendar year contract except for
the total amount of budget which has been increased.
Please let me know if you have any questions. If everything looks okay,
please submit to our County Board. Thank you.
