RE: APPROVE SUPPLEMENTAL STATEMENT OF ASSURANCES AND
CERTIFICATIONS REGARDING 1990 CDBG (COMMUNITY DEVELOPMENT
BLOCK GRANT) PROGRAM AND AUTHORIZE CHAIRMAN TO SIGN
WHEREAS, the Board of County Commissioners of Weld County,
Colorado, pursuant to Colorado statute and the Weld County Home
Rule Charter, is vested with the authority of administering the
affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Supplemental
Statement of Assurances and Certifications regarding the 1990 CDBG
Program, and
WHEREAS, after review, the Board deems it advisable to
approve said Statement, a copy of which is attached hereto and
incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County
Commissioners of Weld County, Colorado, that the Supplemental
Statement of Assurances and Certifications regarding the 1990 CDBG
Program be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chairman be, and
hereby is , authorized to sign said Statement.
The above and foregoing Resolution was, on motion duly made
and seconded, adopted by the following vote on the 2nd day of
July, A.D. , 1990.

\99� ,U� � STATEMENT OF ASSURANCES AND CERTIFICATIONS
1990 Community Development Block Grant Program
The applicant hereby additionally assures and certifies that:
(a) It will comply with the Fair Housing Act (42 USC 3601-20), as
amended, which prohibits housing discrimination on the basis of
race, color, religion, sex, national origin, handicap and familial
status.
(b) In accordance with section 519 of Public Law 101-144, the HUD
Appropriations Act: (check one)

a policy prohibiting the use of excessive force by law enforcement

(c) (1) No Federal appropriated funds have been paid or will be paid,
by or On behalf of the undersigned, to any person for
Influencing or attempting to Influence an officer or employee
of any agency, a Member of Congress, an officer or employee of
Congress, or an employee of a Member of Congress in connection
with the awarding of any Federal contract, the making of any
Federal grant, the making of any Federal loan, the entering
into of any cooperative agreement, and the extension,
continuation, renewal, amendment, or modification of any
Federal contract, grant, loan, or cooperative agreement.
(2) If any funds other than Federal appropriated funds have been
paid or will be paid to any person for influencing or
attempting to influence an officer or employee of any agency,
a Member of Congress, an officer or employee of Congress, or an
employee of a Member of Congress in connection with this
Federal contract, grant, loan, or cooperative agreement, the
undersigned shall complete and submit Standard Form-LLL,
'Disclosure Form to Report Lobbying,' in accordance with its
instructions.
(3) The undersigned shall require that the language of this
certification be included in the award documents for all
subawards at all tiers (including subcontracts, subgrants, and
contracts under grants, loans, and cooperative agreements) and
that all subrecipients shall certify and disclose accordingly.

It is understood that this certification is a material
representation of fact upon which reliance was placed when
this transaction was made or entered Into. Submission of this
certification is a prerequisite for making . or entering into •
this transaction imposed by section 1352, title 31, U.S. Code.
Any person who fails to file the required certification shall
be subject to a civil penalty of not less than $10,000 and not

The attached Supplemental Statement requires County Board approval and
signature. This Supplemental Statement relates to the provisions of the
1989 Housing Act, HUD, and was headed by the Federal Government as a state
requirement after the submission of our Weld County Application. Please
