RE: APPROVE NOTIFICATION FOR PRENATAL PROGRAM CONTRACT RENEWAL WITH THE
COLORADO DEPARTMENT OF HEALTH AND AUTHORIZE CHAIRMAN TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado,
pursuant to Colorado statute and the Weld County Home Rule Charter, is vested
with the authority of administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Notification for Prenatal
Program Contract Renewal between the Colorado Department of Health and the Weld
County Health Department, commencing October 1, 1993, and ending September 30,
1994, with further terms and conditions being as stated in said notification, and
WHEREAS, after review, the Board deems it advisable to approve said
notification, a copy of which is attached hereto and incorporated herein by
reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Notification for Prenatal Program Contract Renewal
between the Colorado Department of Health and Weld County Health Department be,
and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chairman be, and hereby is,
authorized to sign said notification.
The above and foregoing Resolution was, on motion duly made and seconded,
adopted by the following vote on the 11th day of October, A.D. , 1993, nunc pro
tunc October 1, 1993.

The Colorado Department of Health hereby notifies Weld County Health Department
that pursuant to paragraph 10 of contract number PRN930564 , the state will
exercise its option to renew this contract for an additional one year period
beginning October 1, 1993 through September 30, 1994, and awards One Hundred
Thirty-five Thousand Fifty Dollars ($135.050.00) ,
Twenty-eight Thousand Dollars ($28.000) shall be used to administer the progam,
purchase medical supplies and pharmaceuticals and to provide medical, laboratory,
education and counseling to 70 patients; Thirty-three Thousand. Seven Hundred
Fifty Dollars (S33.750) shall be used to provide 450 home visits; Seven
Thousand Dollars ($7.000) shall be for diagnostic tests; Two Thousand Four
Hundred Dollars ($2.400) shall be for pregnancy test; One Hundred-fifty Dollars
($150) shall be for travel to state required meetings; and Sixty-three Thousand
Seven Hundred Fifty Dollars ($63.750) shall be for enhanced services for 300
patients.
Please sign and return all copies of this notification. A fully executed copy
will be returned to you.
• This notification is intended to be effective October 1, 1993 but in no event
shall this amendment be deemed valid until it shall have been approved by the
State Controller or his designee.

COLORADO From John S. Pickle, Director, Weld County Health Dept.
Subset: Prenatal Program Contract Renewal
Enclosed for Board approval is a renewal contract between the Weld County Health
Department and the Colorado Department of Health for our Prenatal Program.
The Health Department will receive $135,050.00 for the period October 1, 1993
through September 30, 1994. The Health Department will provide prenatal care for
Medicaid and non-Medicaid clients. In addition, these funds will be used to
provide 450 prenatal home visits, and enhanced home visiting services for 300
patients.
We would recommend approval of this contract.
