Section 27: Si NW, NW SW, NW NW Weld County, Colorado WHEREAS, said lease is presently operated by Gerrity Oil and Gas Corporation, and WHEREAS, pursuant to Weld County's mineral leasing policy, said authorization must be approved by the Board of County Commissioners, and WHEREAS, after consideration, the Board deems it appropriate to approve said authorization. Enclosed is our Authorization to Receive Oil and Direction for Payment covering your interest. Please examine the Authorization form for accuracy and if you are in agreement with the interest set forth, execute the original, retaining the copy provided for file and future reference. A correct mailing address and Social Security or Tax Identification Number are necessary and should be included in the spaces provided. The executed original should then be returned to our office. If the current ownership or owner's name is different from what is shown on the Authorization, please attach legal documentation to support the change. Your return of the fully executed Authorization form verifies that your interest is correctly stated and continues your interest in a current pay status. Payment for our purchases will be released to you on the 20th day of the month following our purchase, if the accruals to your interest total at least$25.00. Any interest accruing less than$25.00 by year-end, will be released at that time.
