RE: APPROVE CHILD ABUSE CONSULTATION CONTRACTS WITH DR. DONALD COOK AND DR. THERON
SILLS, AND AUTHORIZE CHAIRMAN TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant
to Colorado statute and the Weld County Home Rule Charter, is vested with the
authority of administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with Child Abuse Consultation Contracts
with Dr. Donald Cook and Dr. Theron Sills for the calendar year of 1991, with the
terms and conditions being as stated in said Contracts , and
WHEREAS, after review, the Board deems it advisable to approve said Contracts,
copies of which are attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the 1991 Child Abuse Consultation Contracts be, and hereby
are, approved.
BE IT FURTHER RESOLVED by the Board that the Chairman be, and hereby is ,
authorized to sign said Contracts.
The above and foregoing Resolution was, on motion duly made and seconded,
adopted by the following vote on the 14th day of January, A.D. , 1991.

1. This contract, made this 1st day of January , 19 91 by and between
the Weld County Department of Social Services, PO Box A
Greeley, Colorado 80632 hereinafter called "County" and
Theron Sills, MD. , 2020-16th St. , Greeley, Co. hereinafter called "Contractor".

2. This contract shall be effective fran January 1 , 1991 until
January 1 , 1992 . This agreement may be terminated in
advance of the expiration date specified upon 30 days prior amitten

notice of either party being given to the other party. On termination of
this contract all accounts and payments will be processed according to
financial arrangements set forth herein for services rendered to date of
termination.
3. County agrees to purchase and Contractor agrees to furnish Psychiatric Consultation
(See attachment number l for description of psychiatric
consultation ) to the Cotmtp. p�yrhiatrir rnncultatior will be provided
by the Contractor to the Feld County Child Abuse Team in partial fulfillment .
of the County's statutory duty for coordination of the Team. Payment will
be made to the Contractor at the rate of $ 65:00 per hour
(unit of service)

4. County agrees:
a. To pay Contractor after satisfactory ccapletion of one or more con-
sultations and receipt of the billing form provided by County Voucher
procedure.
b. To assure that the service described herein has been provided
prior to payment to Contractor.
5. Contractor agrees:
a. To provide the service described herein at the location shorn as the
address of the contractor or Weld County Child Abuse Team Meeting, Weld Mental
Health Center, Inc. , 1306-17th Ave. , Greeley, Co. 80631
b. Not to ?ccign any provision of this contract to a s brnatractor.

c. Not to charge clients any fees related to services provided under
this contract.
d. To hold the necessary license(s) which permits the performance of the
service to be purchased herein.
• e: To abide by provision of the Civil Rights Act of 1964 which
prohibits discrimination against any employee or applicant for
-- ecmlovrent or discrimination against any client because of race,
• religion, color or national origin.
f. To cxuply with repuir�.ents for safeguarding client information in
accordance with rules of the State Department of Social Services.
g. Maintain fiscal books, records, documents relevent to this contract
for a period of five years or until a federal audit is completed,
whichever occurs later, after the final payment under this contract.
h. To provide access by county, state and federal officials to records
or other information pertaining to the contractor's provision of
service under this contract.
i. To provide the service described herein at cost not greater than that
charged to other persons in the sane c munity.
6. All terns and conditions are included in the contract; all items
incorporated by reference are physically attached.
This contract contains all the tern. and conditions a=nted upon by the
parties and includes Attachment Number(s) one (1) . All items
incorporated by reference are attached. No other understandings, oral
or otherwise, retarding the subject matter of this agreement, shall be
deemed to mist or to bind any other parties hereto.

The specifics of this contract includes attendance at two (2) Child Protection
Team meetings per month, providing medical and/or psychiatric consultation on
three to four cases per meting.
The contract also includes consultation on crisis situations where a session is
needed to met the special needs of an abused or neglected child.
