RE: APPROVE CONTRACT AMENDMENT WITH GREELEY TRANSITIONAL HOUSE AND AUTHORIZE
CHAIRMAN TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado,
pursuant to Colorado statute and the Weld County Home Rule Charter, is vested
with the authority of administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Contract Amendment between
Greeley Transitional House and the Weld County Department of Social Services,
with the terms and conditions being as stated in said amendment, and
WHEREAS, after review, the Board deems it advisable to approve said
amendment, a copy of which is attached hereto and incorporated herein by
reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, ex-officio Board of Social Services, that the Contract
Amendment between Greeley Transitional House and the Weld County Department of
Social Services be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chairman be, and hereby is,
authorized to sign said amendment.
The above and foregoing Resolution was, on motion duly made and seconded,
adopted by the following vote on the 7th day of June, A.D. , 1993.

CONTRACT AMENDMENT
BETWEEN GREELEY TRANSITIONAL HOUSE AND
WELD COUNTY DEPARTMENT OF SOCIAL SERVICES
THIS AGREEMENT, made and entered into this 27th day of May, 1993, by and between
the Board of County Commissioners, on behalf of the Weld County Department of Social
Services, hereinafter referred to as "Social Services," and "Greeley Transitional
House," hereinafter referred to as the Contractor."
WITNESSETH:
WHEREAS, authority exists in the Law and Funds have been budgeted, appropriated
and otherwise made available and a sufficient unemcumbered balance thereof remains
available for payment in Contract Encumbrance Number FY92-ESG-4001 and
WHEREAS, required approval, clearance and coordination has been accomplished
from and with appropriate agencies; and
WHEREAS, the United States Government, through the Stewart B. McKinney Homeless
Act of 1987, the Stewart B. McKinney Homeless Assistance Amendments Act of 1988, and
Cranston-Gonzalez National Affordable Housing Act of 1990, has established the
Emergency Shelter Grants (ESG) program and has allowed each state to administer such
federal funds to help improve the quality of emergency shelters for the homeless, to
help make available additional emergency shelters, and to help meet the costs of
individuals, so that these individuals have access to safe and sanitary shelters and
supportive services and homelessness prevention services and other types of
assistance to improve their situations; and
WHEREAS, the parties entered into a Contract dated July 8, 1992, Contract Number
FY92-ESG-4001, herinafter called the original Contract, for the performance of
services and work , in consideration for Emergency Shelter Grant Funds; and
WHEREAS, the State of Colorado has awarded an additional allocation to Social
Servies in the amount of $1,000, as governed by the provisions of P.L. 100-404 and
P.L. 100-628; and
WHEREAS, the Social Services has agreed to award said funds to Contractor to
be administered based on the scope of services as specified in the original Contract;
and
WHEREAS, parties to Contract desire to amend the Contract to reflect the change
to the scope of services in the amount of $1,000; and
WHEREAS, all other provisions of the Contract shall remain unchanged.

SUBJECT: Amendment to the 1993 Emergency Shelter Grant Contra a
DATE: June 2, 1993
Attached for Board consideration is an Amendment to the Emergency Shelter
Grant Contract between the Greeley Transitional House and Weld County
Department of Social Services. The Amendment adds $1,000 to the original
Contract amount to provide for operating costs for the Transitional House.
The additional funds were provided by the State Division of Housing to Weld
County. The additional funds must be obligated by June 30, 1993.
