HomeMy WebLinkAbout920197.tiff RESOLUTION RE: APPROVE GRANT APPLICATION FOR FIRST STEPS PROGRAM FROM WELD COUNTY HEALTH DEPARTMENT TO COLORADO CHILDREN'S TRUST FUND AND AUTHORIZE CHAIRMAN TO SIGN WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to Colorado statute and the Weld County Home Rule Charter, is vested with the authority of administering the affairs of Weld County, Colorado, and WHEREAS, the Board has been presented with a Grant Application for First Steps Program from the Weld County Health Department to the Colorado Children's Trust Fund, commencing July 1, 1992, and ending June 30, 1993, with the further terms and conditions being as stated in said grant application, and WHEREAS, after review, the Board deems it advisable to approve said grant application, a copy of which is attached hereto and incorporated herein by reference. NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld County, Colorado, that the Grant Application for First Steps Program from the Weld County Health Department to the Colorado Children's Trust Fund be, and hereby is, approved. BE IT FURTHER RESOLVED by the Board that the Chairman be, and hereby is, authorized to sign said grant application. OTHER INFORMATION OF COMMUNITY NEED AND ISSUES RELATED TO CHILD ABUSE AND NEGLECT PREVENTION EFFORTS: Weld County has a child abuse team coordinated by The Department of Social Services. 90197 CCTF:RFP/92C0M.PR0F 1 PROGRAM DESCRIPTION Amount of money requested: $19,326 Community need and area of the state to be served: The proposed program will build upon the current First Steps Program (see attachments) addressing some of the concerns that case managers have identified in the families with whom they work: low self-esteem, inadequate knowledge of normal child development, poor social support, isolation and inadequate parenting skills. These factors are known to put families more at risk for abuse or neglect. Services will be provided throughout Weld County. Type of program: Services provided will fall into both the primary and secondary prevention category. Parenting education will be offered to all case management clients. In addition support groups will be set up for at risk populations: teen parents and Hispanic Spanish speaking only parents who seem to be more isolated. Program Rationale: The concerns identified under community need will be addressed using two different methods: 1) Home visitation parent education services will utilize the Parents as Teachers (PAT) curriculum developed by the Missouri Department of Education. The curriculum is based on the assumption that all families benefit from support and information on child development which serves a preventive function. This increases families abilities to cope and be independent and enables parents to build confidence in their parenting abilities. The focus of these visits will be to teach child development, positive parenting skills, provide emotional support and increase self- esteem. The current First Steps program schedules only 2-3 home visits in the year following birth. We plan to expand this service to include monthly visits for the 12 months post delivery. At the present time staffing is adequate (2 1/2 case managers) to increased number of visits. Case managers plan to start offering the PAT program as soon as they are trained (sometime this summer) . By January 1, 1993, our present funding will run out. We are requesting funding from the Colorado Children's Trust Fund for the hours that will be spent by the 2 1/2 case managers on the home visitation parent education services from January 1, 1993 - June 30, 1993. 2) Group support meetings will combine the Parent as Teachers curriculum and the DARE to Be You (DARE) (Parent Training Guide) . Both programs promote decision-making, assertiveness, responsibility and self-esteem. Parents will have an opportunity to share experiences, receive peer support, learn through role-modeling and reinforce the importance of their role as parents. 97)3197 2 Group meetings will offer a variety of activities: educational presentations, discussion time and making age appropriate toys. Nutritional snacks will be provided as an incentive for attendance. Child care activities will be provided for older children. To attract the targeted populations, groups will meet in locations that are convenient and settings that are comfortable. Decreasing isolation is a primary goal of group meetings. The First Steps program has a well developed referral system in the community. Case managers currently have 140 active families with whom they could begin to teach positive parenting skills. In addition, community agencies will be notified that First Steps will now accept post partum referrals (for parenting concerns) which in the past we have been unable to do. Target groups: Although the services offered by First Steps are available to all pregnant women in Weld County, clients are primarily low-income single women. The proposed program will offer the home visitation parent education services to all First Steps clients. Group support meetings will target Hispanic and teen parents. Goals and Objectives: Goal I: Participants in the home visitation parent education program will demonstrate increased self-esteem, increased knowledge of child development and improved positive parenting skills. Objective 1: 200 families will participate in the PAT program Objective 2: 90% of the adults participating in the PAT program will demonstrate increased knowledge of child development. Objective 3: 90% of the adults participating in the PAT program will perceive improved self-esteem and parenting abilities. Objective 4: 50 postpartum referrals will be solicited from various community agencies. Goal II: Participants in the group meetings will demonstrate increased self- esteem, decision-making, self-responsibility, assertive communication skills, increased feelings of social support and a decreased sense of isolation. Objective 1: Three support (of 10-20 participants each) groups targeting Hispanic and teen parents will be established in Weld County. Objective 2: 90% of the adults participating in the group sessions will perceive a decreased level of isolation and an increased level of social support. Objective 3: 90% of the adults participating in the group sessions will perceive 926197 3 improved self-esteem, decision making abilities, self- responsibility and assertive communication skills. The program coordinator is a Registered Nurse with a M.S.N. All staff have extensive experience working with expectant and new families. In addition, several of the case managers have extensive experience working with groups. An early childhood specialist will be hired to provide child care activities at the group meetings. VOLUNTEERS: Will be recruited from early childhood education programs in Greeley to help with childcare activities. Volunteers will also be recruited to help provide transportation to group meetings. FACILITIES: The School District 6 Teen-Parent program, Rodarte Center (located in the Hispanic part of Greeley) and the High School in Ault have volunteered space for group support meetings. All of these facilities have space for adult meetings plus child care. 920197 4 Evaluation: Both formative and sunimative evaluations will be implemented incorporating the Parents as Teachers and Dare To Be You evaluation models. The formative evaluation, to be conducted at 6 months, will focus on determining whether the process and procedures are working, project materials are effective and whether participants are satisfied with the services. The summative evaluation, to be conducted at the end of the project, will measure whether the goals and objectives were met. Pre and post tests will be used to measure changes in knowledge base and parenting behaviors. Self-perception scales will be used to measure changes in attitudes/feelings. The First Steps program already has a computer system for tracking client demographics and activities. Program demographics required by The Colorado Children's Trust Fund could easily be incorporated into this system. The First Steps program is funded through December 92, by a grant from the Colorado Trust. We feel confident that if the Colorado Children's Trust Fund will fund the home visitation parent education and group support components as requested, we will be able to secure additional funding from the community and other grants, for the First Steps program to continue. 920197 5 n n PROJECT BUDGET (July 1, 1992 - June 30, 1993) I. The program is supported in concept and practice by the Weld County Community Health Coalition (see attached membership list) and serves all of Weld County. The main purpose of First Steps is to reduce the incidence of low birth weight babies by helping pregnant women access prenatal care and other community resources as early in their pregnancy as possible. Pregnant women are referred into the program by various community agencies. Clients are then followed by case managers who make monthly home visits to support, educate and encourage women to follow through on mutually agreed-upon goals. Families are followed for up to a year after the birth with much less frequent visits. Since July 1, 1991, First Steps has received 230 referrals from 20 different agencies. The case managers are actively following 140 clients. The following client demographic information is available through our data base system. 53% are 19 years of age or younger 51% are Hispanic 39% have completed high school 7% have some college 76% live in Greeley/Evans 77% are single, divorced or separated 83% are unemployed 94% have no insurance 87% did not plan their pregnancies 37% have transportation problems 44% consider their housing situation temporary 50% earn less than $1300 a year (this includes teens who live at home and list their income as 0) Major accomplishments have been the increasing number of first trimester referrals being received, success in finding all clients prenatal care and reducing the number of agency visits a woman has to make to qualify and obtain prenatal care from six to three.
