Because you identified no factual corrections to be made by June 2, 1994, the preliminary report becomes the final report. This report contains confidential information regarding recipients and shall not be released to the public pursuant to Colorado Revised Statute (C.R.S. ) 26-1-114, and applicable Colorado State Department of Social Services Staff Manuals. Lucy Zamora supervises the NAFS unit which consists of seven technicians who carry an average caseload of 350 to 360 cases each. Each technician does their own intake cases daily except on Wednesday morning, and each technician does their own data entry. The case records are divided alphabetically, and automated reports are used to monitor the case loads for equity. This has been the arrangement for at least the last five years. There are two intake sessions daily at 8:15 AM and 10:15 AM, and all clients are asked to attend these orientation sessions whether they are new clients or ongoing clients who have been in the agency before. They think that this allows them to remind the clients of their rights and reporting responsibilities in a non threatening setting. It is their contention that clients do not always pay much attention during their first visit to the agency because they are frequently in somewhat of a crisis situation. They like the Single Purpose Application. The screener at the front desk checks page one for categories, date stamps the application, and refers the client upstairs to the NAFS Unit. The front desk screening function was rotated among all technicians so that they would get an understanding of the various programs and gain a more complete appreciation of each other's roles. Mike Westmoreland attended one of the intake screening sessions. He stated that the presentation was professional and thorough. All technicians had a good feeling about the training they had received and given. They have regular monthly meetings to discuss policy changes and office procedures. They also have impromptu meetings as required and use the phone message system to share policy interpretations. They all have their own manuals and have all attended State New Technician Training and Ongoing Training. Additionally, Weld County has a trainer (Lupe Martinez) who does needs assessment and training. Lucy Zamora & Adrian Esquibel both speak fluent Spanish and English. There has only been one civil rights complaint regarding Food Stamps during the past ten years, and the complaint really involved some other issue, but was referred to FNS because the client's written statement indicated that there had been discrimination. The technicians stated that most of the client phone calls involved questions about income calculation (especially the 4.333 rule) , vehicles and other resources, and student income. Lucy Zamora and the technicians all discussed the error reduction effort in Weld County over the past several years. They stated that the project involved a great deal of team work with each person in each unit contributing something to the process. For example, Caia Berning in the Support Unit pulls the DOLE wage reports & the IEVS reports and gets a hard copy for the technician at recertification. WEDNESDAY 2 23 94 3:00 PM MARGARET HOCKADAY & GWEN GERTSENBERGER This 7 technician Unit was responsible for the AFDC ongoing caseload. They use the single purpose application. The 7 technicians have a case load of 260 to 285 and the entire unit has about 1,200 cases with food stamps. They have weekly unit meetings to discuss policy changes and other needs. She does supervisory case reviews on about 13 to 14 recertifications per technician each month. She also gets some feed back from the hearings officer who reviews case record materials as part of her duties as hearing officer. They do follow up on the IEVS data matches, and most of these cases are referred to the investigative unit. After the investigations unit obtains the appropriate documentation and verification, the case is generally sent to the local level hearings officer for a hearing on intentional program violation. She had several suggestions for improving the administration of the Food Stamp Program. One involved the high lighting of the dependent care field on the CAFSS system to alert the technician that this issue needed to be addressed when the client's employment income was stopped. The second idea was to have the fields for rent and utilities high lighted whenever the client changed address, to alert the technician to check the fact around the shelter deduction. The third thing that she thought would be helpful is to have the list of AFDC FS clients with child support paid through the child support unit printed out by technician before the 15th of each month so that they would have fewer errors involving the $50 child support disregard. THURSDAY 2 24 94 9:00 AM FRED RUTHERFORD SUPERVISOR AFDC INTAKE, CONNIE SANTILLENAS TECH 3, & PAM AGENS TECH 2 This 7 technician Unit was responsible for AFDC intake. They use the single purpose application. Phylis Perez is the lead technician IV. New applications are screened for expedited service by the front desk. Redetermination cases are not screened at the front desk because the technicians have sent applications to them and set up the appointments. Their average caseload had been 40 to 50 cases for the last several months. They do everything that needs to be done to approve or deny a case including interviewing the recipient, obtaining all of the initial verification for a case, assuring that the client meets with the child support units, and making the final decision about the client's level of benefit. These technicians do their own data entry for the COIN system, but send the food stamp data entry to Shereda Brown. They also handle intake for transitional payments, baby care/ kids care, and medical services. They have three IVB Food Stamp Volumes in the Unit. In addition to the State training, they rely on Lucy Zamora and the PA technicians for ongoing training. They track their own IEVS hits and if a case transfers to a second technician, the first technician prints and clears the IEVS data and assures that the information gets to the proper ongoing technician for follow up. They use form 810 to refer most of the new cases to an investigator for a up front home visit. The technicians do their own data entry for assistance payments (COIN) , but Shereda Brown does most of the data entry for the food stamps (CAFSS) . THURSDAY 2 24 94 10:00 AM MARGE CRANDALL SUPERVISOR ADULT UNIT, PAT LAMICA TECH 3, MYRA CANTON TECH 3 The supervisor was out sick so the two technicians were interviewed instead. Myra had 15 years experience and Pat had 2 years experience. There are 5 technicians in the unit plus 1 nursing home technician. There are no lead technicians, but they have requested a reclassification of one of the technician positions. The cases are divided alphabetically and have to have a money payment to be included in this unit. Myra had 700 OAP cases including 80 to 90 with food stamps. Pat had about 100 cases with food stamps. The technicians do some interviews by phone if it is difficult for the client to appear in the office due to age or some physical limitation. There is a data entry clerk who does the food stamps data entry. They have monthly Unit Meetings, and each technician has a copy of the Food Stamp Regulations. They have both attended State Technician training as well as their own internal training. The Supervisor handles complaints, and has a log of those complaints and their resolution. Technicians are responsible for documentation of any complaints on the case record application or report of contact form. The supervisor performs supervisory case reviews and monitors the IEVS overdue list to assure that technicians are obtaining verification of this information. There are occasional cases requiring claims or investigative referral, but this is a less frequent occurrence than it is in some of the other categories. FRIDAY 2 25 94 10:00 AM DAVE HUFFMAN - CHIEF INVESTIGATOR He supervises two field investigators, Bob French and Dave Lawley. Bob French investigates cases south of highway 34, and Dave Lawley investigates the cases north of Highway 34. There is a referral form that eligibility units use to refer cases to the investigations unit. The form lists 6 reasons for case referral: 1. Unreported income 2. Persons of opposite sex living together - mostly AFDC 3. Household composition reports 4. Forgery 5. Eligibility investigation 6. Food Stamp 7. Front end investigations The duties include follow up on pre-certifications, fraud referrals, preparation for IPV hearings for local level hearings, State administrative hearings, and preparation for Court hearings to be presented by the local District Attorney. They also serve legal documents to obtain information that has not been forthcoming on first requests. The investigators also appear and testify at Court Hearings or Administrative Hearings. They do pre-certification investigations on all AFDC FS intake cases. Dave Hufman keeps extensive daily and monthly logs of all referrals, front end investigations, and fraud investigations. The two investigators are in the field a great deal of the time, but they report their activities to him on a daily log. He screens the referrals before assigning the cases to one of the investigators, because the unit receives more referrals than can actually be handled in any given month. The referrals that are not immediately assigned are turned back to the eligibility unit through the administrator Richard Rowe. There is frequent negotiation about these cases between the investigations unit and the eligibility units, and some of the cases are referred a second time after additional development of the referral by the technician. This process allows the investigations unit to operate efficiently without developing to large a backlog. Dave Huffman also is responsible for completion of the Report on Investigations Activities (form 36A) . Janet Tappy provides some of the data for this report. He also coordinates with Jessica Vogelgesang in her reviews of IEVS earnings reports. While there appear to be several minor problems in completion of the form 36A, it is the reviewer's observation that this is one of the most productive units in the State. The unit is viewed as an integral part of the eligibility and certification process. There is a real working emphasis on team work and coordination. FRIDAY 2 25 94 10:00 AM INTERVIEW RICH ROWE, DAVE HUFFMAN, JAN TAPPY, JESSICA VOGELGESANG RICH ROWE - ASSISTANCE PAYMENTS ADMINISTRATOR & FRAUD PREVENTION UNIT SUPERVISOR Rich Rowe administers the Assistance Payments Unit, the Investigations Unit, and the Fraud Prevention Unit. He developed a strong error rate reduction plan. His case load reports, and the corrective action plan for the food stamp unit is attached. He explained that he had each individual and each unit develop a plan for their contribution to the error reduction plan. These plans are similar to other plans the reviewer has seen on paper, but the process of developing these plans may be somewhat different in the sense that each person seemed knowledgeable about the plan and each unit seemed quit committed to doing what they could to improve payment accuracy. The NA Unit did a 100% case review of all cases over a period of time, and committed one investigative technician to doing the follow up on the IEVS data matches after the technician reviewed the information against the case record. They focussed on error prone cases. They improved communication between units. They put a strong emphasis on front end investigations and follow up on other investigations and claims activities. They also had some training on regulations, documentation & verification, and improved communication between units. The result of these effort was the lowest case and payment error rate of any of Colorado's ten large counties. Mr. Rowe also said that he had expressed the concern that client service would remain a priority for the agency, as well as, the emphasis on payment accuracy. Jessica Vogelgesang, who reports to Rich Rowe, investigates IEVS wage referrals for the food stamps program. She gets these reports from the technicians after they have done their preliminary case reviews. She has good organizational skills and follows up on each of the IEVS data matches by writing and calling employers, and sending legal documents and doing field work as needed. She keeps a file on each case she investigates, so that she does not need to do additional work for those cases that are referred more than once. She also appears to testify for the local level IPV hearings as needed. Local Level Hearings Officer Janice C. Tappy, also reports to Rich Rowe. In addition to her duties as local level hearings officer, she also reviewed a large number of error prone food stamp cases as part of a 100% review of all food stamp cases in Weld County. In addition to reviewing cases that were probable error cases, she was able to develop information regarding training needs for the eligibility supervisors and administrator. Janice Tappy used one of the cases selected for review (Jesus Valdez) as a model for explaining the procedures she uses in her role as IPV hearings officer. On 4/23/93 she received a request for an IPV hearing because the client had not reported earnings received from 5/92 through 10/92. On 4/27/92 she sent the client a notice of the scheduled IPV hearing, an explanation of hearings procedures, a waiver form, a letter explaining the client's rights, and a list of free attorney's. On 6/25/93 she remailed these same forms to the client's new address. The letters were sent by both regular mail and registered mail. She received no response within 15 days. She also sent a notice of the hearing to the agency personnel regarding the hearing. On 8/19/93 She held the hearing and sent out notices regarding her decision about intentional program violation. On 9/14/93 she entered the data regarding the six month disqualification on the automated system. Complaints Coordinator - Ju_y Crosbey - Secretary to the Director of Weld DSS. She serves as the complaints coordinator for Weld County. All personnel were aware of the need for a complaint log, and referred to her role as complaints coordinator when this topic was discussed. She discussed her role as complaints coordinator for the County DSS with the reviewer. She tracks the complaints that come through the Director' s Office, She says that she only had about 2 to 3 FS compaints come through her office during the past 6 months. Two of the complaints were from the same person. Most of the FS complaints are directed to the FS Supervisor or one of the other supervisors if the case is a PA case. She and the supervisors maintain a complaint log. TUESDAY 2 22 94 1:30 PM & WEDNESDAY 2 23 94 9:OO AM ISSUANCE REVIEW DALE LENGEL Dale Lengel assisted the reviewers with the review of issuance and security. In addition, he answered the questions regarding these two subjects and provide the reviewers with a procedural overview of this operation. The County has implemented a strong corrective action plan to reduce mail loss and improve security. Physical changes were made to the issuance area to improve security and additional changes were being planned. The County Director met with the local police and they did a thorough security review of the facility and made recommendations for additional improvements in security. Since Mr. Lengel was relatively new to the food stamp program, the reviewer mailed some relevant agency letters regarding food stamp issuance. WELD COUNTY APPOINTMENT LOG CASES TO BE PULLED FOR MANAGEMENT EVALUATION December 3 , 1993 1. Lynn Chrisman The client never showed for her appointment. There was no case to review by ME Reviewer. The agency was applying $254 monthly SSI . Effective 3-94 the agency began applying $446 SSI and $34 monthly earnings. An SDX Register shows that on transaction date 12-21-93 the SSI amount is $446 and there is an Earned Income amount of $34 . The client declared a monthly rent amount of $210 on the SPA and attached is a rent verification from Dale and Beth Gainsvelle. The agency is not allowing any shelter costs. The case is timely processed, but it is an error. The agency did document that for December the client would only be receiving $254 SSI . December 9, 1993 5 . Nancy Hicks--523928476 CASE TIMELY PROCESSED/APPROPRIATE CHANGES WERE MADE This client was on the Appointment Log for December 9 . The current certification period is for 9-93 through 2-94 . The ME Reviewer is not sure why the client was on the Appointment Log. The client reapplied on 8-9-93 and was certified on 8-25-93 . Beginning 9-93 the agency was applying $300 monthly earnings, $329 monthly UCB, and $100 Child Support. The client's total expenses were $623 . On 10-29-93 the agency received a Change Report Form in which the client reported a change of address as of October 18 and that her UCB had run out. The client indicated she had moved in with her Mother. The agency made a note at the bottom of the page that the client was still receiving UCB. The agency requested verification of the move. On 12-2-93 the agency received verification that the client paid no rent, but was paying her Mother' s entire utilities. Effective for 1-94 the agency deleted the previous rent amount and was only allowing the SUA. The agency made the change of address on the FS-3B. On 1-10-94 the client reported another move, and the agency made the verified changes in shelter costs. 6 . Kelli Dill The client never showed for her appointment. The SPA shows that a David is applying for Social Security and SSI , however the ME Reviewer does not show a David in the household? The head of the household' s name is Paul or Pablo. The household 's expenses exceed their income, and it appears from the case record that the client has declared no income since January/1993 . The agency was applying $367 monthly gross earnings and $100 other income. The total shelter costs were $494 . Effective 1-94 the agency deleted the earnings and began applying $280 AFDC. The agency continued to apply the $100 other income. The client reapplied on 2-1-94 and the agency is now applying $75 weekly earnings, $140 AFDC, and the $100 other income. December 14 , 1993 9 . She does not qualify as a separate household from her Mother. Her Mother is not aged or disabled. December 3 , 1993 13 . Sheila Schockly--524943265 The client was on the Appointment Log for December 3 . There was not any Food Stamp information in the case file. This case was a Medical only case and was discontinued 12-31-93 . The ME Reviewer checked CAFFS and found that this client had been on food stamps from 8/93 through 1/94 with her husband Travis and their three children. So there was no reason to process food stamps at the time of the review. The absence of the food stamp file was not explored or explained. 14 . Ed Hernandez--522393917 CASE PROPERLY DENIED The client was on the Appointment Log for December 3 . The previous certification period was for 10-93 through 11-93 . The SPA is dated 11-9-93 . FS-3B #18 shows that the client was denied for 12-93 for household composition ineligibility. The case had been referred to the Fraud Unit on 12-9-93 . Apparently Investigations had disclosed that the client 's spouse was living in the household and she had earnings. The case was properly denied. AFDC APPOINTMENT LOG CASES December 7 , 1993 15. Lavonne Wood--507423125 CASE TIMELY PROCESSED/ ERROR CASE--SHELTER COSTS ARE VENDORED AND BEING APPLIED AS EXPENSES The client was on the December 7 Appointment Log to be given an ID card by the technician. The most recent FS-3 in the file is dated 5-3-93 for a certification period of 6-93 through 5-94 . The client was recertified on 5-17-93 . The agency is applying $36 PA income, $389 Social Security income, and $65 SSI. The client ' s total expenses are $476. On the FS-3 the agency documented an amount of $83 . 01 for the trailer and $190 for the trailer space. There are receipts attached to the FS-3 to verify these amounts. This totals to $273 . The FS-3B shows the agency applying $278 . The FS-3B shows the agency is also allowing the Telephone Standard of $24 . On the FS-3 there is a notation that the client' s son, Mike, is paying this expense. If the payment is vendored than the client should not be allowed the expense. The other possible error in this case is that on the receipt log for the trailer space rent amount of $190 the Landlady states that for 5-1 and 6-1 the $190 was paid by Mike. Again this is a vendored payment and should not be allowed as an expense. The Landlady also indicates that there are two adults in the household. If the client ' s son, Mike, is the other adult and he is in fact paying the space rent and the telephone charges, than the client should not be allowed those expenses. 16 . On an AP-9, dated 1-24- 94 , the agency indicated that the client expects to receive a lump sum settlement from an Insurance Co. due to a car accident in 1989 . The settlement is being held in a Trust Account until the client turns 21 on 1-18-94 . The agency indicated that the case was certified for December and January only. The date of the interview on the SPA was 12-9-93 . FS-3B record #2 shows that the case was certified on 12-15-93 . The case has -0- income and was not certified for expedited services. AFDC APPOINTMENT LOG CASES December 9 , 1993 17. Caroline Madison--522577435 ACTION REQUIRED - FS APPLICATION WAS NOT APPROVED OR DENIED. This client was on the December 9 Appointment Log. On the SPA the client indicated she was applying for Food Stamps, AFDC, Baby Care/Kids Care, and Medical Assistance. The client indicated she was living with her Sister. Her Sister' s date of birth is 6-11-53 . The client does not qualify for separate household status from her Sister, as the Sister is not aged or disabled. The agency opened the case for Medical Assistance only. The ME Reviewer did not find a Denial Notice on the client in the case file. The ME Reviewer looked at the CAFFS screen and found no record of the client using her name or her social security number. Since the client did apply for food stamps, there should have been a denial of her food stamps even if she did not qualify. December 13 , 1993 18. Paul Clifford--396768085 CASE IMPROPERLY DENIED FOOD STAMPS The client was on the December 13 Appointment Log. On the SPA, dated 12-13-93 , the client applied for Food Stamps, AFDC, Baby Care/Kids Care, Leap, and Medical Assistance. The client declared that the household' s last income was received 12-1-93 . The agency notes that the client 's last day of work was 12-6-93 and his last check of $600 included vacation pay. The client declared a rent amount of $550 and utility expenses. There were two pay-stubs in the case file that totalled to $1670. 46 for December. The case was approved 2-9-94 for Medical only. The agency indicated that the Kids Care was approved for two children only. Apparently the Food Stamps were never approved as the agency indicated that the husband never cooperated by going to Job Services. The problem is that as long as the client Work Registered, the case should have been opened for Food Stamps by 1-13-94 . On 1-7-94 the agency indicated that her husband was still looking for employment and that if he found employment she would report it. From the date of application to 1-13-94 the client was unemployed, and the case could have been opened for Food Stamps by 1-13-94 . The client' s subsequent failure to cooperate with Job Search would have been an action to be considered after 1-13-94 . December 14 , 1993 19. The due date was 1-3-94 . The most recent FS-3 is dated 10-29-92 , for a certification period of 11-92 through 4-93 . The previous cert. periods were 5-92 through 8-92 and 9-92 through 10-92 . For the cert. period 5-92 through 8-92 the agency was applying UCB for the client ' s spouse, Eleuterio Chavez . The total amount being applied was $836 each month.
