RE: APPROVE LETTER FOR IMMUNIZATION CONTRACT RENEWAL WITH THE COLORADO
DEPARTMENT OF HEALTH AND AUTHORIZE CHAIRMAN TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado,
pursuant to Colorado statute and the Weld County Home Rule Charter, is vested
with the authority of administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Letter for Immunization
Contract Renewal between the Colorado Department of Health and Weld County Health
Department, commencing October 1, 1993, and ending September 30, 1994, with
further terms and conditions being as stated in said letter, and
WHEREAS, after review, the Board deems it advisable to approve said letter,
a copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Letter for Immunization Contract Renewal between the
Colorado Department of Health and Weld County Health Department be, and hereby
is, approved.
BE IT FURTHER RESOLVED by the Board that the Chairman be, and hereby is,
authorized to sign said letter.
The above and foregoing Resolution was, on motion duly made and seconded,
adopted by the following vote on the 11th day of October, A.D. , 1993, nunc pro
tunc October 1, 1993.

Subject: Immunization Contract Renewal
Enclosed for Board approval is a renewal contract between the Weld County Health
Department (WCHD) and the Colorado Department of Health.
The amount of the contract is the same as Fy 92, $59,996, for participation in
a state comprehensive plan to increase and enhance the availability of age -
appropriate immunizations for children. WCHD will utilize these funds to meet
specific immunization goals for children in Weld County.
This program focus on low immunization rates among a critical public health
population, the 2 to 4 year old age group. We would recommend your approval of
this contract, and continuation of this program.
