RE: APPROVE AMENDED PURCHASE OF SERVICES AGREEMENT WITH WELD FOOD BANK AND
AUTHORIZE CHAIRMAN TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado,
pursuant to Colorado statute and the Weld County Home Rule Charter, is vested
with the authority of administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with an Amended Purchase of Services
Agreement between the Board of County Commissioners of Weld County, Colorado, on
behalf of the Weld County Department of Social Services, and the Weld Food Bank,
commencing October 1, 1993, and ending September 30, 1994, with the further terms
and conditions being as stated in said agreement, and
WHEREAS, after review, the Board deems it advisable to approve said
agreement, a copy of which is attached hereto and incorporated herein by
reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, ex-officio Board of Social Services, that the Amended Purchase
of Services Agreement between the Board of County Commissioners of Weld County,
Colorado, on behalf of the Weld County Department of Social Services, and the
Weld Food Bank be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chairman be, and hereby is,
authorized to sign said agreement.
The above and foregoing Resolution was, on motion duly made and seconded,
adopted by the following vote on the 6th day of October, A.D. , 1993, nunc pro
tunc, October 1, 1993.

AMENDED PURCHASE OF SERVICES AGREEMENT BETWEEN
THE BOARD OF COMMISSIONERS OF WELD COUNTY, ON BEHALF
OF THE WELD COUNTY DEPARTMENT OF SOCIAL SERVICES AND THE WELD FOOD BANK
THIS AMENDMENT, made and entered into this 6th day of October, 1993, by
and between the Board of Commissioners of Weld County, Colorado, on behalf of
the Weld County Department of Social Services, hereinafter referred to as
"WCDSS", and the Weld Food Bank, hereinafter referred to as "Contractor."
WITNESSETH:
WHEREAS, THE WCDSS and the Contractor have entered into a Purchase of
Services Agreement (Contract No. PY93-TEFA-01) for the local distribution of
USDA commodities through the Temporary Emergency Food Assistance Program, and
WHEREAS, WCDSS and the Contractor desire to modify its original
Agreement, dated January 15, 1993, in order for the Agreement to coincide
with the federal fiscal year, and
WHEREAS, the required approval, clearance, and coordination has been
accomplished from and with appropriate agencies.
NOW, THEREFORE, in consideration of the mutual promises and covenants
contained herein, the parties hereto agree as follows:
1. This Amendment shall be effective October 1, 1993. The term of
the Amendment shall commence upon its execution and shall remain
in effect for a period of one (1) year and nine months, beginning
January 1, 1993, through September 30, 1994.
2. Except as modified herein, the terms and conditions of the
original Agreement, (PY93-TEFA-01) , and modification (PY94-TEFA-
02) , together with all attachments hereto, remain in full force
and effect.
IN WITNESS WHEREOF, the parties hereto have duly executed the Amendment
as of the day, month, and year first above written.

Services and the Weld County Food Bank '
DATE: September 28, 1993
Enclosed for Board approval is an Amendment to a Purchase of Services
Agreement between the Board of Commissioners of Weld County, on behalf of the
Weld County Department of Social Services, and the Weld Food Bank.
The purposes of this Amendment are:
1. Continue to have the Weld Food Bank administer the Temporary Emergency
Food Assistance Program (TEFAP) . Their primary purpose is to
distribute donated foods to various Weld County agencies.
2. Adjust the ending date of the original Agreement of December 31, 1993,
to September 30, 1994. In order to have this agreement coincide with
the same contract time period established between Weld County and the
State Department of Social Services, the contract period will be
extended to September 30, 1994. This change will convert the Purchase
of Services Agreement. with Weld Food Bank to be on a federal fiscal
year (October 1, 1993 through September 30, 1994) .
