RE: APPROVE AFFILIATION AGREEMENT BETWEEN HEALTH DEPARTMENT AND UNIVERSITY OF
COLORADO HEALTH SCIENCES CENTER AND AUTHORIZE CHAIRMAN TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado,
pursuant to Colorado statute and the Weld County Home Rule Charter, is vested
with the authority of administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with an Affiliation Agreement between
the Weld County Health Department and the University of Colorado Health Sciences
Center, with the terms and conditions being as stated in said agreement, and
WHEREAS, after review, the Board deems it advisable to approve said
agreement, a copy of which is attached hereto and incorporated herein by
reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Affiliation Agreement between the Weld County Health
Department and the University of Colorado Health Sciences Center, be, and hereby
is, approved.
BE IT FURTHER RESOLVED by the Board that the Chairman be, and hereby is,
authorized to sign said agreement.
The above and foregoing Resolution was, on motion duly made and seconded,
adopted by the following vote on the 11th day of December, A.D. , 1991.

Enclosed for Board approval is an affiliation agreement between the Weld
County Health Department and the University of Colorado Health Sciences Center
(UCHSC) .
This agreement outlines the two agencies' responsibilities in the use of the
Weld County Health Department as a training site for residents in the UCHSC
Preventive Medicine Residency Program.
This is a continuation and refinement of the current affiliation agreement
between the agencies.

WELD COUNTY HEALTH DEPARTMENT
Program Director:
William M. Marine, M.D. , M.P.H. will direct and coordinate
the program's activities in all participating institutions
assuring that the contributions of the affiliated
institutions facilitate the accomplishment of the overall
educational mission of the program.
Program Goals and Objectives:
To become the education and training resource in Colorado
and the Rocky Mountain region for general preventive
medicine specialists with broad training in both public
health practice and clinical preventive medicine.
Site Goals and Objectives:
To provide a resident with exposure to a "full-service"
local health department. There is a breadth of
opportunities in which a resident can become involved, and
Weld County's diversity provides a wide range of potential
involvement including agricultural injury prevention, access
to health in rural areas, effectiveness of emergency medical
services in rural areas, and a multitude of environmental
issues. The resident also has access to a clinical family
medicine residency training program and the opportunity to
look at primary health care services and training of
physicians.
1. EDUCATIONAL OBJECTIVES
The following are the UCHSC competency-based educational
objectives that can be addressed at this affiliated
institution: (See attached list)

Additional educational opportunities addressed by this
affiliated institution are as follows:
a. Communicate epidemiologic principles and methods to
public health employees.
b. Understand the principles of effectively communicating
risk to public and elected officials.
c. Understand how to access the public health system
including state and federal agencies.
2. SCOPE OF THE AFFILIATION:
Rotations of six and twelve months are available and provide
training suitable to public health track preventive medicine
residents.
The specific opportunities available for residents to
accomplish the educational objectives of the rotation are
listed according to the basic components of preventive
medicine as follows:
a) Epidemiology and Biostatistics
i. Resident will assume the position of epidemiologist
for the health department with tasks including:
-Biostatistical analysis and general evaluation of
various programs including community education,
environmental, occupational, and prenatal care projects
(Weld County Prenatal Care project, Groundwater nitrate
study of National Hog Farms as well as county-wide
groundwater system, Community/minority cardiovascular
disease prevention education program) ;
-Location, organization, and graphical interpretation
of county data in order to assist in the development of
strategic goals and objectives;
-Generation, preparation and dispersal of requested
data to appropriate member of the county community;
-Provision of epidemiological consultation to North
Colorado Family Medicine Residency Program (to include
investigation, identification, and evaluation of
factors associated with retention of alumni in rural
areas) .

b) Health Administration
i. Observation of and participation in various health
administration, quality assessment, personnel and
fiscal management committee proceedings at the county
and state levels.
ii. Specific examples of committee organization/
involvement include:
-Weld Safety Network-Board Member-data provision and
evaluation development;
-Colorado State Cardiovascular Disease Prevention
Coalition, Screening Subcommittee, Special Population/
Minority Section-planning and development;
-Weld County Health Department Administration -
observation of directorship and management;
-Colorado Public Health Administration - observation;
-Colorado Health Education Council - observation;
-Colorado Health Officers Association - observation;
-Weld County Health Department Sexually Transmitted
Disease and Family Planning clinics - clinical
participation.
c) Occupational/Environmental Health
i. Observation and assistance in the development of
strategic goals of Weld County Health Department
Environmental Protection Division activities and
programs.
ii. Observation of an environmental specialist on
various inspections and fieldwork operations.
iii. Participation in Weld Safety Network.
d) Cultural and behavioral factors
i. See ai. and bi. above.
e) Clinical applications of preventive medicine
i. Preventive clinical consultation to Family
Practice residents at the North Colorado Family
Practice residency program.
ii. Investigation, evaluation and intervention, as
needed, of infectious outbreaks/epidemics, at the
Weld County Health Department.

3. RESOURCES AVAILABLE TO RESIDENTS (space, support
services and clinical facilities)
Residents participating in a rotation at Weld County will be
provided desk space, phone and secretarial support,
reference books, and access to the public health library and
the medical library at the hospital. They will be provided
a PC terminal with basic software installed such as dBase,
Lotus, WP, EPI5, Wonder, and they will have access to
clinical databases from the Health Department and to
residency program Medline.
4. DUTIES AND RESPONSIBILITIES OF RESIDENTS AT AFFILIATE
First year residents will have academic responsibilities on
Mondays and Wednesdays but will be at the practicum sites on
Tuesdays, Thursdays and Fridays. Second year residents and
practicum year only residents will be essentially full time
at the practicum site except for additional required and
elective coursework and other residency-related activities.
5. RELATIONSHIP BETWEEN RESIDENTS, STAFF OF THE RESIDENCY
PROGRAM, AND THE AFFILIATE INSTITUTION
Affiliate institutions will interview residents interested
. in working in their agency. Residents will be assigned to
agencies based on approval from the practicum site director,
approval from the program director, the resident's training
track (public health or clinical preventive medicine) ,
training needs, and the preferences of the resident.
Within the first two months at the practicum site the
program director will meet with the practicum site director
and resident to review and agree upon activities of the
resident for the rotation. Activities will be linked with
the educational objectives to be addressed during the
rotation.
Practicum site directors will evaluate residents at the end
of the rotation or every six months. Residents will
evaluate the faculty and the practicum site experience at
the end of the rotation or every six months. The program
director will do an overall evaluation of the resident's
progress every six months.

FINANCIAL COMMITMENT:
The Weld County Health Department agrees to attempt lato attemp their

commitment will be reviewed and reaffirmed annually on March
1 for the academic year commencing in 4 months on July 1 .
Graduate Medical Education funds from Weld County Health
Department will be used to support the entire mission of the
preventive medicine residency training program-

This agreement remains in effect indefinitely by mutual
agreement, but can be dissolved by either party given
sufficient notice in keeping with the financial commitment
section of this agreement.
