PROJECT: CENTENNIAL DEVELOPMENTAL SERVICES BUILDING PLUMBING WORK
TO (CONTRACTOR) : CORMAN MECHANICAL, INC.
JUSTIFICATION: Change order removal of sink from janitors to handicapped
area and install baseboard in boys & corridor in amount of $408.20 &
handicapped drinking fountain in amount of $300.00 - total $708.20.
You are directed to make the following changes in the work. All other terms
and conditions of the contract not expressly modified hereby shall remain in
full force and effect.
The original contract sum was $ 13,960.00
Net change by previous option $ 7, 716.41
The contract sum prior to this Change Order was $ 21,356.41
The contract sum will be (increased) or
(unchanged) by this Change Order $ 708.20
The contract sum including this Change Order
will be $ 22,064.61
The new contract time will be (increased) (decreased) or (changed) by
( 0 ) days.
The date of completion as of the date of this Change Order is therefore
August 31 , 19 92.
