LATION.
TO (CONTRACTOR) : MACKENZIE MANUFACTURING
JUSTIFICATION: Change orders for additional cubbies and installation
in the amount of $3,675.02; $2, 064.47; $713.63; and $317.50 in the total
amount of $6, 770.62. Added one classroom of cabinets plus three diaper
changing areas.
You are directed to make the following changes in the work. All other terms
and conditions of the contract not expressly modified hereby shall remain in
full force and effect.
The original contract sum was $ 33,156.50
Net change by previous option $ -0-
The contract sum prior to this Change Order was $ 33, 156.50
The contract sum will be (increased) or
