HomeMy WebLinkAbout940331.tiff RESOLUTION RE: APPROVE SPECIAL PROJECTS CONTRACT BETWEEN NORTH COLORADO MEDICAL CENTER FOUNDATION AND HEALTH DEPARTMENT AND AUTHORIZE CHAIRMAN TO SIGN WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to Colorado statute and the Weld County Home Rule Charter, is vested with the authority of administering the affairs of Weld County, Colorado, and WHEREAS, the Board has been presented with a Special Projects Contract between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Weld County Health Department, and North Colorado Medical Center Foundation, commencing March 1, 1994, and ending February 28, 1995, with further terms and conditions being as stated in said contract, and WHEREAS, after review, the Board deems it advisable to approve said contract, a copy of which is attached hereto and incorporated herein by reference. NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld County, Colorado, that the Special Projects Contract between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Weld County Health Department, and North Colorado Medical Center Foundation be, and hereby is, approved. BE IT FURTHER RESOLVED by the Board that the Chairman be, and hereby is, authorized to sign said contract. Kirkm er 940331 , „ c! `. . l/r - � j'=' .. , II l c,n /ln MED-AID PRESCRIPTION ASSISTANCE PROGRAM PROVIDER INFORMATION Med-Aid is a joint funding effort of the North Colorado Medical Center Foundation and United Way of Weld County to address acute medication needs of patients unable to pay and having no access to any other funding source for prescription assistance . Participating providers include North Colorado Medical Center Emergency Room, Sunrise Community Health Center, North Colorado Family Medicine, Plan de Salud Del Valle, Inc. , Cottonwood Pediatric Associates, Greeley Medical Clinic Pediatric Department, and the Weld County Health Department Clinics . Med-Aid is a pilot program and depends upon proper administration of the following guidelines to enable the program to continue : 1 . Screen for financial need. Patient should have no access to any other funding source for prescriptions . Med-Aid is considered the funding of las resort . 2 . Refer only Weld County residents . Program is funded by donations from private citizens for use by Weld County residents . 3 . Order only those drugs on the accepted Med-Aid formulary. Wherever possible, generic meds will be used. 4 . Covers one prescription per person, but no refills . Med-Aid use is restricted to single acute use versus chronic care. 5 . Work with pharmacists to identify any recommendations for revisions in the formulary. North Colorado Medical Center Foundation and United Way of Weld County will review the formulary based on the utilization of the existing meds and request from providers for alternative meds . 6 . Stamp the from of the original prescription with the "MED- AID SEE REVERSE" stamp. 7 . Stamp the reverse of the original prescription with the Large Stamp. 8 . Initial the "approval" signifying the patient qualifies . 9 . Complete the "Birthdate" information. This information will be used for evaluation of the program effectiveness . 10 . Refer the patient to K Mart Greeley Pharmacy, 2829 West 10th Street or Plan de Salud Del Valle, Inc . , pharmacy, 1115 2nd Street, Fort Lupton. North Colorado Medical Center Pharmacy will only fill prescriptions for North Colorado Medical Center Emergency Room patients and provide after hours (9 p.m. to 9 a.m. ) coverage for K Mart . 940331 NORTH COLORADO MEDICAL CENTER FOUNDATION SPECIAL PROJECTS CONTRACT Contract between North Colorado Medical Center Foundation, 1801 16th Street, Greeley, Colorado, and Weld County Health Department Clinics, 1517 16th Avenue Court, Greeley, Colorado. North Colorado Medical Center Foundation agrees to reimburse K Mart Pharmacy #4347, 2829 West 10th Street, Greeley, Colorado, for medications provided to those who are determined by a financial screening by Weld County Health Department Clinics to be unable to pay for an acutely needed medication. This grant program will commence on March 1, 1994 and will continue until February 28 , 1995 or until funds have been dispersed or until either of the parties notifies the other in writing that the program is canceled. This contract shall be in effect from March 1, 1994 to February 28, 1995 . This program, know as the Med-Aid Prescription Program, is co- funded by United Way of Weld County, Inc . Purpose of this funding is to enable Weld County residents, unable to afford medications, to obtain single prescription financial assistance for acute medical needs . Weld County Health Department agrees to the following: 1 . Abide by the terms of this "Special Projects Contract" and with the Med-Aid guidelines (attached) . 2 . Screen for financial need. Patient should have no access to any other funding source for Rx. Med-Aid is considered the funding of last resort . 3 . Refer only Weld County residents . 4 . Prescribe only drugs on the accepted Med-Aid formulary (attached) . 5 . Prescribe single Rx only. 6 . Work with pharmacists to identify any recommendation for revisions in the formulary. North Colorado Medical Center Foundation agrees to: 1 . Administer the Med-Aid program and work with the Med-Aid advisory Committee to set policy and formulate program changes . 2 . Consider revisions to the formulary as recommended by Weld County Health Department Clinics and participating pharmacists . 3 . Ensure the program meets the acute prescriptions needs of indigent Weld County residents . 94V31.
