RE: APPROVE PURCHASE OF SERVICES AGREEMENT FOR USAGE OF COMPUTER EQUIPMENT
BETWEEN EMPLOYMENT SERVICES AND SCHOOL DISTRICT RE-5J AND AUTHORIZE
CHAIRMAN TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado,
pursuant to Colorado statute and the Weld County Home Rule Charter, is vested
with the authority of administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Purchase of Services Agreement
for Usage of Computer Equipment between the County of Weld, State of Colorado,
by and through the Board of County Commissioners of Weld County, on behalf of
Employment Services, and School District RE-5J, commencing June 6, 1994, and
ending August 31, 1994, with further terms and conditions being as stated in said
agreement, and
WHEREAS, after review, the Board deems it advisable to approve said
agreement, a copy of which is attached hereto and incorporated herein by
reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Purchase of Services Agreement for Usage of Computer
Equipment between the County of Weld, State of Colorado, by and through the Board
of County Commissioners of Weld County, on behalf of Employment Services, and
School District RE-5J be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chairman be, and hereby is,
authorized to sign said agreement.
The above and foregoing Resolution was, on motion duly made and seconded,
adopted by the following vote on the 1st day of June, A.D. , 1994.

USAGE OF COMPUTER EQUIPMENT PROJECT
PURCHASE OF SERVICES AGREEMENT

EFFECTIVE The effective date of this Agreement is April 1, 1994 through
DATE: August 30, 1994.
It is the intent to use the stand-alone computers to assist in
providing remedial activities for Job Training Partnership Act
In-School Youth. The four stand-alone computers will be
housed at the Roosevelt High School in Johnstown, Colorado
during the term of this agreement.
It is also the intent of the parties to use these computers to
provide academic enrichment for participating youth in the
1994 Summer Youth Employment and Training Program.
PURPOSE: The duration, terms and conditions for an agreement of
services between Weld County School District RE-5J (SDRE-5J)
and Employment Services of Weld County are being established
to provide access, primarily to JTPA eligible program
participants, to the Jostens Learning System.
AMENDMENTS: This Agreement may be amended at any time with the written
consent of all parties. ESWC may issue policies and
procedures, unilaterally in writing, to Weld County School
District RE-5J to enhance, clarify, or alter the conditions of
equipment usage.
FINANCIAL ESWC will not charge SDRE-5J usage fees for the four stand-

SDRE-53 shall be responsible for all deductible dollar amounts
in regard to insurance fees in the case of theft, vandalism,
burglary, and other such cause-related damages or losses of
above mentioned equipment. All consumable supplies required
in the operation or usage of this equipment shall be provided
by SDRE-5J.

TERMINATION: This Agreement may be terminated immediately by ESWC if
funding is no longer available to support the Project, or if
SDRE-5J fails to comply with the terms of this Agreement.
GEOGRAPHIC Jostens Learning System Computers/Programs will be housed

Job Training Partnership Act eligible youth who are in need of
remedial education activities.
AGREEMENT: This Agreement shall remain in effect as outlined under
"Effective Date", is subject to the availability of funds, the
provisions listed above, and as specified in the Terms and
Conditions, and will be reviewed by both parties as specified
in the Terms and Conditions of this Agreement. It is the
intent of both parties to review the agreement for the next
school year.
I. TERMS AND CONDITIONS
A. Description of Equipment
Equipment consists of, four (4) stand-alone computers with CD-ROM,
voice synthesizers, head sets, printers and Jostens Invest Software.
B. School District RE-5J Responsibilities
The following responsibilities shall be assigned to School District
RE-5J in the implementation of this Project Agreement.
1. Supervision of Jostens Learning System when in use.
2. Reliable security system installed to prevent user's access to
the CD-ROM Disc.
3. Purchase of all consumable supplies required when using the
Jostens Learning System.
4. Payment of all deductible insurance fees due to vandalism,
burglary, theft, etc.
5. General upkeep of equipment, i.e. cleaning of equipment
(screens, keyboard) . This shall include the use of
appropriate cleaners, prohibition of food or drink in close
vicinity of equipment, appropriate storage of discs, etc.
The financial responsibilities for replacement of any
equipment damaged through negligence, i.e. computers are
dropped and broken, malfunction due to dropping food or
liquids on the equipment, or other general negligence.

6. Provision of two (2) staff persons for training and
supervisory purposes on System.
Staff who are carried over from the normal school year to
supervise and train youth in the lab during the Summer Youth
Employment and Training Program will be reimbursed wages by
Employment Services.
7. Assurance of priority usage of System by JTPA eligible
clients.
8. Complete and submit to ESWC case manager appropriate
documentation for each youth enrolled in JTPA within one week
of youth's utilization of the System.
9. Refer youth to ESWC, when appropriate, to complete JTPA
applications.
10. Follow-up with each youth and verify services provided.
11. Maintain a complete file of all records, documents,
communications, and other materials which pertain to the
operation of the System and delivery of services under this
Agreement.
12. Provide appropriate reports as developed jointly be ESWC and
SDRE-5J.
13. The School District shall assume all responsibility in the
regard to security and safeguarding of said equipment.
14. Provide quarterly in-kind financial statements to ESWC.
15. SDRE-5J understands that approximately every 45 days, the
computers will need to be returned to ESWC for updates and
maintenance.
C. Employment Services of Weld County Responsibilities
Employment Services shall be responsible for the following in the
implementation of this Project Agreement.
1. Provide technical expertise and support of the Jostens
Learning System and equipment.
2. Provide updates of System software.

3. Provide all maintenance and cleaning of System hardware. Due
to contractual agreement between ESWC and Jostens, all
equipment shall be transported to the Human Resources building
for any updates, maintenance, cleaning or repairs.
4. Maintain all contracts and insurance policies for each stand-
alone computer.
5. Provide training to two (2) school staff persons who will
provide continual supervisory guidance to individuals
utilizing the System.
Reimburse staff wages for those school staff who are carried
over from the normal school year to provide training and
supervision for youth enrolled in the Summer Youth Employment
and Training Program.
6. Provide appropriate feed-back to school staff in regard to
Project requirements.
7. Assist in the development and maintenance of records for all
JTPA eligible individuals utilizing the System for the purpose
of accurate billing to appropriate funding sources.
8. Participate with appropriate school personnel in the
development of appropriate reports in regard to the outcomes
of the Project and assist, as required, in the completion of
the reports.
II. Modification of Agreement
All modifications to this Agreement shall be in writing and signed by both
parties.
III. Termination
This Agreement may be terminated at any time by either party given thirty
(30) day written notice and is subject to the availability of funding.

IV. Assignment
This Agreement shall be binding upon the parties hereto, their successors,
heirs, legal representatives, and assigns. School District RE-5J may not
assign any of its rights or obligations hereunder without the prior
written consent of Employment Services.

The purpose of this agreement is to outline the terms and conditions in
which Roosevelt School District will utilize the Employment Services Jostens
stand alone computer system located at Roosevelt High School.
This system provides for academic enrichment and remediation for JTPA
eligible youth.
During the summer months Employment Services will pay the salaries
of Roosevelt High School teachers designated to deliver these services to
youth in the Sumner Youth Employment Program.
The amount for teacher salaries will not exceed Four Thousand, Five Hundred and
Ninety-Three Dollars ($4,593.00) from June 6, 1994, through August 31, 1994.
