RE: APPROVE AMENDMENT TO AGREEMENT FOR SHERIFF'S OFFICE JAIL MEDICAL UNIT
SERVICE WITH PREFERRED CORRECTIONAL MEDICAL TEAM, INC. AND AUTHORIZE
CHAIRMAN TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with an Amendment to Agreement for Sheriff's
Office Jail Medical Unit Service between the County of Weld, State of Colorado, by and through the
Board of County Commissioners of Weld County, on behalf of the Weld County Sheriff, and
Preferred Correctional Medical Team, Inc., commencing January 1, 1995, and ending December 31,
1995, with further terms and conditions being as stated in said amendment, and
WHEREAS, after review, the Board deems it advisable to approve said amendment, a copy
of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Amendment to Agreement for Sheriffs Office Jail Medical Unit Service
between the County of Weld, State of Colorado, by and through the Board of County Commissioners
of Weld County, on behalf of the Weld County Sheriff, and Preferred Correctional Medical Team, Inc.
be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chairman be, and hereby is, authorized
to sign said amendment.
The above and foregoing Resolution was, on motion duly made and seconded, adopted by
the following vote on the 12th day of December, A.D., 1994.

AMENDMENT TO
AGREEMENT FOR WELD COUNTY SHERIFF'S OFFICE
JAIL MEDICAL UNIT SERVICE
This Amendment, dated for reference purposes only the 31st day
of December, 1994, is made and entered into by and between the Board
of County Commissioners of Weld County, Colorado, ( "County" ) and
Preferred Correctional Medical Team, Inc. ( "Provider" ) . It is
hereby attached to and shall become a part of the Agreement by and
between the County and Provider which was executed September 11,
1988.
WITNESSETH :
WHEREAS, the County and Provider entered into a Medical Unit
Service Agreement ( "Agreement" ) on September 11, 1988; and
WHEREAS, said Agreement has been renewed and extended by
Amendments through December 31, 1994; and
WHEREAS, said Amendments set the monthly payment at 839, 020. 00
for the period through December 31, 1994; and
WHEREAS, the parties hereto agree that there is a need for an
increase in said monthly payment amount for the period January 1,
through December 31, 1995.
NOW, THEREFORE, for good and valuable consideration, the
receipt and sufficiency of which is hereby acknowledged, the County
and Provider agree to the following:
1. That the monthly payment amount of 839, 020. 00 is hereby
adjusted to 840, 500. 00 for the period from January 1 through
December 31, 1995.
2. That the Agreement, as amended hereby, shall remain in
full force and effect through December 31, 1995.
IN WITNESSETH WHEREOF, t parties have executed this
amendment the /A7lt day of 1994.
