RE: APPROVE AMENDMENT TO COORDINATION AGREEMENT BETWEEN SCHOOL DISTRICT RE-7
AND FENWC AND AUTHORIZE CHAIRMAN TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado,
pursuant to Colorado statute and the Weld County Home Rule Charter, is vested
with the authority of administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with an Amendment to Coordination
Agreement between School District RE-7 and Family Educational Network of Weld
County, commencing October 15, 1992, and ending June 31, 1993, with the further
terms and conditions being as stated in said amendment, and
WHEREAS, after review, the Board deems it advisable to approve said
amendment, a copy of which is attached hereto and incorporated herein by
reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Amendment to Coordination Agreement between School
District RE-7 and Family Educational Network of Weld County be, and hereby is,
approved.
BE IT FURTHER RESOLVED by the Board that the Chairman be, and hereby is,
authorized to sign said amendment.
The above and foregoing Resolution was, on motion duly made and seconded,
adopted by the following vote on the 19th day of October, A.D. , 1992, nunc pro
tunc October 15, 1992.

AMENDMENT TO THE COORDINATION AGREEMENT BETWEEN SCHOOL
DISTRICT RE-7 AND FAMILY EDUCATIONAL NETWORK OF WELD COUNTY
THIS AGREEMENT, made and entered into this 19th of Oct. , 1992, between
the County of Weld, State of Colorado, by and through the Family Educational
Network of Weld County, hereinafter referred to as "FENWC" and School District
RE-7, hereinafter known as "School District RE-7".
WITNESSETH:
WHEREAS, the parties desire to continue their coordination agreement for
services rendered and received, and
WHEREAS, the parties desire to modify their original contract of June 1,
1992, and
WHEREAS, required approval, clearance, and coordination has been
accomplished from and with the appropriate agencies.
NOW THEREFORE, in consideration of the premises, the parties hereto
covenant and agree as follows:
1. This amendment shall be effective October 1, 1992.
2. School District RE-7 acknowledges that this Agreement shall be reviewed for
renewal and possible revision each program year.
3. Provision 4, (School District RE-7 Responsibilities) , Section G, of the
original agreement will be revised as follows:
G. a. School District RE-7 will provide breakfast and milk for
approximately thirty (30) preschool children at the cost of One
Dollar and Forty Cents ($1.40) per child/adult.
School District RE-7 will provide lunch and milk for approximately
thirty (30) preschool children at the cost of One Dollar and Eighty
Cents ($1.80) per child/adult.
School District RE-7 will provide snack and juice for approximately
thirty (30) preschool children at the cost of Seventy-Five Cents
($.75) per child/adult.
b. School District RE-7 agrees to provide information on the cost of the
quantity of lunches served to enable FENWC to comply with USDA
reimbursement requirements.

c. School District RE-7 agrees to provide meals in accordance with the
regulations of the United States Department of Agriculture's Child
Care Food Program as shown in Attachment A hereto, which is hereby
incorporated into this Agreement.
d. School District RE-7 will maintain the following records for FENWC:
1. Menu/Productions Records, including number of meals prepared ,
menu, portion sizes of menu items and amounts of food used to
prepare menu items. This is a requirement at the preparation
kitchen for all meals served.
2. Food and milk invoices.
e. FENWC will apply directly to USDA for reimbursement for each child's
meal by type (full paid, free or reduced) each month. School
District RE-7 will not receive USDA reimbursement for these meals.
FENWC agrees to pay School District RE-7 for all meals served to Head
Start children and staff. Such reimbursement will be made within
three (3) weeks of the receipt of a statement of such charges from
School District RE-7. The rate will be as described in item 3, a. ,
above.
4. Provision 4, (School District RE-7 Responsibilities) , of the original
agreement will be revised by addition of the following Section:
I. School District RE-7 will provide regular (home to school and school to
home) pupil transportation for approximately thirty (30) preschool
children at a cost of Two Dollars and Fifty-Five Cents ($2.55) per day
an average of 8.5 days per month not to exceed Three Thousand Dollars
($3,000) per year. The rates will be reviewed annually.
5. Except as modified herein, the terms and conditions of the original Agreement
(Coordination Agreement Between School District Re-7 and Family Educational
Network of Weld County) , together with all attachments hereto, remain in full
force and effect.

of day, month, and year, first above written.

Enclosed for Board approval is an Amendment to the Coordination Agreement between
School District RE-7 and Family Educational Network of Weld County.
The parties desire to continue their coordination for services rendered and
desire to modify their original contract of July 1, 1992.
