RE: APPROVE LETTER OF RENEWAL FOR CONTRACT FOR CHILD HEALTH PROGRAM BETWEEN
HEALTH DEPARTMENT AND COLORADO DEPARTMENT OF PUBLIC HEALTH AND ENVIRONMENT
AND AUTHORIZE CHAIRMAN TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado,
pursuant to Colorado statute and the Weld County Home Rule Charter, is vested
with the authority of administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Letter of Renewal for the
Contract for the Child Health Program between the County of Weld, State of
Colorado, by and through the Board of County Commissioners of Weld County, on
behalf of the Weld County Health Department, and the Colorado Department of
Public Health and Environment, commencing October 1, 1994, and ending
September 30, 1995, with further terms and conditions being as stated in said
letter, and
WHEREAS, after review, the Board deems it advisable to approve said letter,
a copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Letter of Renewal for the Contract for the Child
Health Program between the County of Weld, State of Colorado, by and through the
Board of County Commissioners of Weld County, on behalf of the Weld County Health
Department, and the Colorado Department of Public Health and Environment be, and
hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chairman be, and hereby is,
authorized to sign said letter.
The above and foregoing Resolution was, on motion duly made and seconded,
adopted by the following vote on the 19th day of October, A.D. , 1994, nunc pro
tunc October 1, 1994.

Dear Contractor:
This is to notify you that pursuant to Paragraph 15 of Contract WCH940672,Routing Number 951400,the allocation
of funds for the period beginning October 1, 1994 and continuing through September 30, 1995 is thirty-five thousand,
one hundred seventy-three dollars ($35,173), to be spent in the following manner: $3,434 per month for the first 9
months and$4,267 total for the last 3 months of the period 10/1/94 through 9/30/95.
This contract includes $18,114 which represents your share of the total Maternal and Child Health funds appropriated
by the legislature to local health departments for the period July 1, 1994 to June 30, 1995. Receipt of these funds
requires local match of$13,586,and is subject to audit.
The Contractor shall provide child health services to a minimum of 572 unduplicated clients between 10/1/94 and
9/30/95.

Subject: Child Health Program Renewal
Enclosed for Board approval is a letter of renewal for the contract between Weld County Health
Department (WCHD) and the Colorado Department of Public Health and Environment for the
Child Health Program.
WCHD will provide well child clinic services to a minimum of 572 unduplicated clients 0-16
years of age. The services will include health history screenings, physical exams, immunizations
and referrals to appropriate agencies if necessary.
For these services, WCHD will receive a total reimbursement of$35,173 which is a decrease of
$408 over the prior year's contract. This decrease is consistent with the decrease in number of
clients to be seen from 600 to 572. The contract includes $18,114 which represents the WCHD's
share of the Maternal and Child Health funds appropriated by the legislature to local health
departments.
The term of the contract is from October 1, 1994 through September 30, 1995.
I recommend your approval of this letter of agreement.
