RE: APPROVE AGREEMENT BETWEEN COMPRECARE FOUNDATION, INC. AND THE WELD COUNTY
HEALTH DEPARTMENT AND AUTHORIZE CHAIRMAN TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado,
pursuant to Colorado statute and the Weld County Home Rule Charter, is vested
with the authority of administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with an agreement between the
Comprecare Foundation, Inc. and the Weld County Health Department, commencing
Januay 1, 1993, and ending December 31, 1993, with the further terms and
conditions being as stated in said agreement, and
WHEREAS, after review, the Board deems it advisable to approve said
agreement, a copy of which is attached hereto and incorporated herein by
reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the agreement between the Comprecare Foundation, Inc. and
the Weld County Health Department be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chairman be, and hereby is,
authorized to sign said agreement.
The above and foregoing Resolution was, on motion duly made and seconded,
adopted by the following vote on the 30th day of December, A.D. , 1992.

AGEZEMENT
THIS AGREEMENT, made and entered into by and between the
CLARE FOUNDATION, INC., hereinafter called the "Foundation," and
the WELD COUNTY HEALTH DEPARTMENT, hereinafter called the "Grantee,"
sets forth the terms and conditions of the grant by the Foundation to
the Grantee as is more fully stated below.
I. Terms of Grant.
Grantee: Weld County Health Department.
Date Authorized: November 30, 1992.
Amount: $10,000.00
Specific Purpose: TO develop a program to increase community awareness
of suicide prevention issues among the elderly, increase knowledge
about completed suicides in Weld County, and develop other sources of
support for these activities, in accordance with the grant requests of
August 13, 1990, October 8, 1991, and September 8, 1992, which are
incorporated by reference herein.
Grant Period: From January 1, 1993, through December 31, 1993.
II. Review of Grant Activity.
Grantee �l pprovide the Foundation with quarterly reports
throughout the term of the and grant, with the first n urt due April 1,
1subsequent reports due every three months thereafter. Unless
otherwise requested by the Foundation, such reports shall not exceed
two pages in length. At the completion of the project, Grantee shall
provide the Foundation a summary report which outlines the general
nature of the work performed and provides a detailed accounting of
funds expended.
Grantee further agrees to advise the Foundation of any
schedule changes delays, or conditions that may impact the scheduled
completion of this project.
III. Special Provisions.
This grant is for the specific purposes set forth above, and
the funds provided by the Foundation may be used only in accordance
with those purposes. Any modification of these purposes can be made
only with the Foundation's prior written approval.
No funds provided pursuant to this grant may be used to
participate in or intervene in any political campaign or to support any
attempt to influence legislation through (1) an attempt to affect the
opinion of the general public or any segment thereof or (2)
canmmication with any member or employee of a legislative body or with

any government official or emmployee who may participate in formulation
of legislation, : other than through making available the results of
nonpartisan�is, study, and research.

this grant and for maintaining adeqnate supporting records consistent
with generally accepted accounting practices.

Grantee shall return to the Foundation any unexpended funds
at the close of the grant period. Furthermore, the entire amount of
the grant shall be returned if the Foundation determines that Grantee
has not performed in accordance with the terms of this agreement or if
Grantee loses its exemption under Section 501(c) (3) of the Internal
Revenue Code of 1954, as amended.
Grantee shall provide the Foundation with copies of all its
policies of nondiscrimination in enployment for its employees,
nondiscrimination concerning the use of funds which it distributes, or
affirmative action for its employees.

Subject Comprecare Foundation Inc. Agreement
Enclosed for Board approval is an agreement between the Comprecare Foundation
Inc. and the Weld County Health Department.
The agreement outlines the activities for the grant award of $10,000 to be used
to develop a program to increase knowledge of suicides and suicide prevention in
Weld County.
The grant period shall be from January 1, 1993 through December 31, 1993.
