RESOLUTION RE: APPROVE ANNUAL PLAN AND GRANT APPLICATION, TITLE III, OLDER AMERICANS ACT, BETWEEN AREA AGENCY ON AGING AND COLORADO DEPARTMENT OF SOCIAL SERVICES, AGING AND ADULT SERVICES AND AUTHORIZE CHAIRMAN TO SIGN P WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to Colorado statute and the Weld County Home Rule Charter, is vested with the authority of administering the affairs of Weld County, Colorado, and WHEREAS, the Board has been presented with an Annual Plan and Grant Application, Title III, Older Americans Act, between Area Agency on Aging and Colorado Department of Social Services, Aging and Adult Services, commencing January 1, 1994, and ending December 31, 1994, with further terms and conditions being as stated in said application, and WHEREAS, after review, the Board deems it advisable to approve said application, a copy of which is attached hereto and incorporated herein by reference. NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld County, Colorado, that the Annual Plan and Grant Application, Title III, Older Americans Act, between Area Agency on Aging and Colorado Department of Social Services, Aging and Adult Services, be, and hereby is, approved. BE IT FURTHER RESOLVED by the Board that the Chairman be, and hereby is, authorized to sign said application. Kirkmeyer 931093 STATEMENT OF ASSIISAYCES The following list identifies new, deleted, or amended assurances which now must be addressed in all area plans in addition to those assurances detailed in the current Four Year Plan. 1. The area agency assures that Title III funds will not be used to pay any part of a cost (including administrative costs) incurred by the AAA to carry out a contract or commercial relationship that is not carried out to implement Title III. Sec. 306(a)(15) 2. The area agency assures that preference in receiving Title III services will not be given to any individual as a result of a contract or commercial relationship that is not carried out to implement Title III. Sec. 306(a)(16) 3. The area agency assures that projects will reasonably accommodate participants described in sec. 307(a)(13)(G) of the Act, that is, persons with particular dietary needs. Sec. 306(a) (17) 4. The area agency assures, to the maximum extent practicable, to coordinate the services provided under Title III with services provided under Title VI. Sec. 306(a)(18) 5. The area agency assures that it will pursue activities to increase access by older individuals who are Native Americans to all aging programs and benefits provided by the agency, including programs and benefits under Title III and will specify the ways in which the area agency intends to implement the activities. Sec. 306(a)(19) 6. The area agency assures that providers will serve low-income minority individuals in accordance with their need for service. Sec. 306(a)(5) 7. The area agency assures that each activity undertaken by the agency, including planning, advocacy, and systems development will include a focus on the needs of low-income minority older individuals. Sec. 306(a) (5) 8. The area agency assures that any Title III amount received under Part D, Part G, Part F, and Special Ombudsman Activity funds will be expended in accordance with such part, and will not be used to supplant any other Federal, State, or local funds that would otherwise be expended for such purposes. The area agency assures that all activities of the AAA, whether funded by public or private funds, conforms with the responsibilities of the area agency and conforms with the laws, regulations, and policies of the State. Page_of_ AAS 120 (7/93) Page 1 931093 SECTION B: ANNUAL PLAN Region IIB FY 94 ANNUAL PLAN SUMMARY Based on the goals identified in the Four Year Plan, briefly describe the overall direction to be taken by the Area Agency on Aging toward the development of a comprehensive, coordinated system of services for older persons in the region. Include a description of all of the Area Agency's activities, whether funded by public or private funds. Include, also, any changes, deletions and/or additions of goals and objectives. This annual plan is the last plan in the 1991 - 1994 planning cycle. The Weld Area Agency on Aging, in conjunction with the Area Agency Advisory Board Members, has reviewed our progress in meeting the goals as outlined in the Four Year Plan. We submit the following summary which is intended to be a guide to the Area Agency and its board members for actions to be taken in 1994 and as documentation of progress to date. Each of the goals stated in the Four Year Plan are listed below followed by a listing of activities or accomplishments that occurred in 1993 to meet the goal. The Area Agency will work toward the implementation of a county - wide single entry point for long term care services On September 29, 1993 the Weld County Area Agency on Aging received approval to become a Single Point of Entry Agency for Long Term Care Services for adult citizens of Weld County. The start up date for this program will be February 15, 1994. Since July, 1989 the Area Agency on Aging has managed the Home and Community Based Services Program which has served, on average, sixty five individuals at any given time. This home based care program will be expanded to include the Home Care Allowance Program, Adult Foster Care, and Certification for Nursing Home Care under the Single Point of Entry Program. The commitment of the Area Agency to home based care within the long term care continuum has increased significantly during the current four year planning cycle. Assist the Weld Health Coalition to bring additional monies into the county for health research, health education, and health prevention programs for Weld seniors. The Area Agency Director has been a member of the Weld Health Coalition for the past three years. The Coalition applied, in 1993, for several grants to the Colorado Trust and other foundations. Currently the coalition has an application under review for the Colorado Healthy Communities Initiative. Page 2 921093 Page Two Annual Plan Summary A Request for Proposal under Title III, Part F of the Older Americans Act has been publicized to the Weld community for health promotion projects. The Area Agency on Aging Advisory Board will make recommendations for funding for FY 1994 for one or more health promotion projects. Increase informational, educational, and advocacy activities on behalf of Weld seniors. In 1993 the Area Agency initiated a one page newsletter which is made available to all Senior Nutrition Sites. The newsletter prints articles informing seniors of programs and services for seniors, as well as, information about the senior nutrition program. This newsletter will be distributed on a quarterly basis. In 1993, the Area Agency Senior Legal Aide Program recruited eight local, private attorneys who agreed to accept referrals from the Area Agency and charge reduced rates for services to appropriately eligible seniors. This programs increases the opportunity for legal assistance, particularly to those seniors who fall outside of the Area Agency guidelines. In 1993, an Ensure Plus Food Supplement Program was started as an adjunct service of the Weld County Senior Nutrition Program. The Ensure Plus is made available to seniors who demonstrate need by bringing a doctor's prescription to the Area Agency Office at the time of registration. To date, fifty (50) frail elders have taken advantage of this service. The Area Agency conducted two, six week pre retirement planning classes in 1993 for North Colorado Medical Center. Planning is underway to offer this workshop to City of Greeley Employees. These classes are a cooperative venture with the Weld County Extension Office using the AARP "Think of Your Future" program. Conduct a major needs assessment to better determine what seniors themselves believe their needs to be. In 1992 the Area Agency conducted a combination evaluation/needs assessment of the Weld County Senior Nutrition Program with the assistance of Robbyn Wacker, Ph.D. and Assistant Professor at the Gerontology Program, University of Northern Colorado. A Caregiver Initiative Grant from the Administration on Aging has allowed the Area Agency to coordinate a community caregiver project in the communities of Frederick, Firestone, and Dacono in South Weld County. A community survey has been completed. Data analysis is in progress. An action plan for meeting the needs of the community will be written by November 30, 1993. Page 3 9:'•1093 Page Three Annual Plan Summary A survey of the Weld rural Senior Center Programs is in the preliminary planning stages and will be conducted in conjunction with a Doctoral student from the University of Northern Colorado Gerontology Program. This survey is to be completed by December, 1993. The Area Agency will initiate partnerships that will address the serious need of additional resources to provide drugs, eye care, dental care, and hearing aides on an emergency basis. An Area Agency staff member actively participates in the Weld Hearing Aide Bank Advisory Board. This board makes recycled hearing aides available to eligible persons. Meetings with United Way, North Colorado Medical Center, Sunrise Health Clinic, and the Family Practice Residency Program have not generated hoped for emergency resources to purchase needed drugs for seniors. The problem centers around the fact that this is such a large problem and one which cannot be solved without health care reform. Agencies and the private sector are reluctant to take a band aide approach to this major problem. The Area Agency will seek additional avenues for providing information and support for a constantly increasing group of caregivers. The Area Agency continues to participate in the Public Service Gatekeeper program. Referrals are limited. Caregiver informational packets are mailed to families on request. A one page Senior Information and Referral List was designed and printed by the Area Agency and is distributed throughout the county to individuals and organizations. This listing is updated regularly. Senior Companions and Respite Care Programs are supported through a contract with Catholic Community Services Northern. The Area Agency coordinates articles on caregiving and other services which are written by agency representatives. These articles appear in the Weld County Extension Newsletter which is mailed to 2000 families county wide. Work for increased employment opportunities for older adults desiring to remain in the work force. The Area Agency continues to sponsor an Older Worker Program funded under the Job Training Partnership Act. Job counseling, training, and placement was provided to 33 older workers. The 1994 JTPA contract has been approved for $37,500 to serve 25 older workers. The targeted number is fewer due to changes in the federal guidelines regarding eligibility for this program. The Older Worker Specialist became a certified AARP Works Coordinator in 1993. Two Workshops were conducted for 30 individuals to train them on resume writing, interviewing skills, and on building confidence for job seeking. Page Four Annual Plan Summary Work for increased meaningful opportunities in the community for older adults with increased emphasis on increasing opportunities for volunteerism in the public school systems. In March of 1993, the Area Agency conducted its first annual Leadership Program for older adults. This eight week program is designed to prepare older adults for community involvement as volunteers, board members, and legislative advocates. Twenty older adults participated in this pilot project. The second course will be offered in March, 1994. The Area Agency on Aging submitted a grant application for the Colorado Public Service Company Intergenerational Projects in 1993. This grant was written in conjunction with the Greeley Senior Activity Center, United Way, School District #6, and the Retired Senior Volunteer Program. The proposal was not funded; however the concepts in the proposal are still being explored and the grant supporters continue to meet to find ways to expand senior involvement in the schools. The Area Agency Director solicited and accepted membership on the United Way Volunteer Resources Committee. The committee provides oversight for the School Volunteer Project. The Area Agency provides advice and technical assistance on matters related to expansion of recruitment of older adults for the School Volunteer Project. Targeting to Minority include minority service levels in all purchase of service agreements. (1993 Objective Met, Same for 1994) provide training and technical assistance to AAA Board members, Senior Aide Coordinators, Long Term Care Coordinating Committee, and others on issues and concerns related to serving minorities. (Same for 1994) seek culturally relevant training for all AAA staff (Same for 1994) include minority service levels in all purchase of service agreements (1993 Objective Met, Same for 1994) utilize some of the AOA caregiver grant to assess needs in north Greeley (primarily Hispanic) in cooperation with the City of Greeley Planning Department. (1993 Objective not met) maintain funding and support of the Catholic Community Services Elderly Hispanic Outreach Program. The B Services cover a number of various services to Weld County's elderly population. Because of the size and various services given in this category, Weld County Area Agency on Aging feels the need to supply this section with extra dollars, as long as it does not inhibit service to any other component. The Weld County Area Agency on Aging will make its recommendations on $346,790 in federal and state funding to local programs for adult day care, home delivered meals, home health care, peer counseling, respite care, and outreach and nutrition services. Funding is made available under the Older Americans Act. Community members will be given the opportunity to voice concerns and suggestions to assure that community needs for the elderly are being addressed. Page 28 9:^1.99' Summary of Public Hearing A public meeting was held October 12, 1993 in Hudson, Colorado in the community room of the Prairie View Senior Housing Complex. Press releases were sent to local area newspapers and posters were placed in public areas. Fourteen individuals were present to hear an overview of the Weld County Area Agency on Aging and the funding recommendations for the 1994 Annual Plan. Each member present was asked to comment on the plan or on other concerns they would like to address. The following is a summary of the comments that were recorded: A majority of the members present expressed concerns about the lack of transportation to the area. Participants need a demand/response system that will get them to doctor's appointments when they are unable to schedule too far in advance. Options were discussed, including accessing a limited amount of funds to reimburse volunteer community drivers. Several participants supported the concept of having the Area Agency on Aging paralegal have office hours in south Weld County to make this service more accessible to seniors who are unable to travel to Greeley. One participant was concerned about the Flu shot schedule for Weld County. Transportation was also an issue for this participant. The meeting commenced at 10:30 a.m. and adjourned at 12:00 noon. Weld County AAA has purchase of service agreements with two agencies for grants greater than $25,000. Every two years the Fiscal Department conducts on site assessments of each sub grantee and contracted agency to examine financial transactions, accounts, reports, and assess compliance with applicable regulations. The most recent audit of Title III providers was conducted in November, 1993. Weld County AAA does request annual audit reports from each sub grantee and contract agency with whom we have purchase of service agreements.
